目的:探讨2型糖尿病(T2DM)合并代谢相关脂肪性肝病(MAFLD)患者血清CD47、FK506结合蛋白5(FKBP5)、X盒结合蛋白1(XBP1)表达及临床意义。方法:将收治的106例T2DM合并MAFLD患者设为T2DM合并MAFLD组,另将118例单纯T2DM患者设为T2DM组。酶联免疫吸附试验(ELISA)检测血清CD47、FKBP5、XBP1水平;CD47、FKBP5、XBP1水平与糖脂代谢指标、空腹胰岛素(FINS)及稳态模型胰岛素抵抗指数(HOMA-IR)的相关性采用Pearson法分析;T2DM患者发生MAFLD的影响因素通过多因素Logistic回归分析;CD47、FKBP5、XBP1诊断T2DM患者合并MAFLD的效能及临床效益分别通过受试者工作特征(ROC)曲线及临床决策曲线(DCA)分析。结果:与T2DM组比较,T2DM合并MAFLD组T2DM病程较长,空腹血糖(FPG)、糖化血红蛋白(HbA1c)、FINS、HOMA-IR、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)水平较高,高密度脂蛋白胆固醇(HDL-C)水平较低,血清CD47、FKBP5、XBP1水平均较高(均P<0.05)。T2DM合并MAFLD患者血清CD47、FKBP5、XBP1水平与FPG、HbA1c、FINS、HOMA-IR、TG、LDL-C存在一定相关性(均P<0.05)。T2DM病程、HbA1c、FINS、HOMA-IR、TG、LDL-C、CD47、FKBP5和XBP1与T2DM患者MAFLD的发生密切相关(均P<0.05)。CD47、FKBP5、XBP1联合诊断T2DM合并MAFLD的敏感度和特异度分别为82.08%和86.44%,且曲线下面积(AUC)高于CD47、FKBP5、XBP1单项诊断(均P<0.05)。DCA分析也表明联合诊断具有较高临床价值。结论:血清CD47、FKBP5、XBP1与T2DM合并MAFLD患者的糖脂代谢及胰岛素抵抗(IR)密切相关,三项联合对T2DM合并MAFLD具有良好的诊断价值。

Objective:To explore the expression and clinical significance of serum CD47,FK506-binding protein 5 (FKBP5),and X-box binding protein 1 (XBP1) in patients with type 2 diabetes mellitus (T2DM) complicated with metabolic associated fatty liver disease (MAFLD).Methods:The 106 patients with T2DM and MAFLD were set as the T2DM complicated with MAFLD group,another 118 patients with simple T2DM were set as the T2DM group.ELISA was used to detect serum CD47,FKBP5 and XBP1.The correlations between the CD47,FKBP5,XBP1 levels and glucose and lipid metabolisms indicators,fasting insulin (FINS),and homeostatic model assessment of insulin resistance (HOMA-IR) were explored by the Pearson method.The influencing factors of MAFLD in patients with T2DM were explored by multivariate Logistic regression.The efficacy and clinical benefits of CD47,FKBP5 and XBP1 in diagnosing patients with T2DM complicated with MAFLD were analyzed by ROC curve and decision curve analysis (DCA),respectively.Results:Compared with the T2DM group,the T2DM complicated with MAFLD group had a longer course of T2DM,higher fasting plasma glucose (FPG),glycated hemoglobin (HbA1c),FINS,HOMA-IR,triglycerides (TG),and low-density lipoprotein cholesterol (LDL-C),and lower high-density lipoprotein cholesterol (HDL-C),and higher serum CD47,FKBP5 and XBP1 (all P<0.05).Serum CD47,FKBP5,and XBP1 in patients with T2DM complicated with MAFLD were correlated to some extent with FPG,HbA1c,FINS,HOMA-IR,TG,and LDL-C (all P<0.05).Course of T2DM,HbA1c,FINS,HOMA-IR,TG,LDL-C,CD47,FKBP5,and XBP1 were closely related to the occurrence of MAFLD in patients with T2DM (all P<0.05).The sensitivity and specificity of the combined diagnosis of CD47,FKBP5,and XBP1 in T2DM patients complicated with MAFLD were 82.08% and 86.44%,respectively,and the AUC was higher than that of the individual diagnosis of CD47,FKBP5,and XBP1 (all P<0.05).DCA analysis also indicated that the combined diagnosis had high clinical value.Conclusion:Serum CD47,FKBP5,XBP1 are closely related to glucose and lipid metabolisms and IR in patients with T2DM complicated with MAFLD.The combination of these three indicators has a good diagnostic value for T2DM combined with MAFLD.
