摘要:
目的:探讨动脉瘤性蛛网膜下腔出血(aSAH)患者血清Dickkopf-1(DKK-1)、烟酰胺腺嘌呤二核苷酸磷酸氧化酶2(NOX2)及可溶性髓样细胞触发受体样转录因子-1(sTLT-1)水平变化及检测意义。方法:选取230例接受血管内介入治疗的aSAH患者为aSAH组,另选100名体检健康者为对照组。根据介入术后6个月改良Rankin量表(mRS)评分将患者分为预后良好组(mRS 0~2分,168例)和预后不良组(mRS 3~6分,62例)。采用ELISA法检测血清DKK-1、NOX2、sTLT-1水平。通过多因素Logistic回归分析aSAH患者介入术后预后不良的影响因素。绘制受试者工作特征(ROC)曲线评估各指标及其联合检测对aSAH患者介入术后预后不良的预测效能。结果:aSAH组血清DKK-1、NOX2、sTLT-1水平高于对照组(均P<0.05)。预后不良组中Hunt-Hess分级Ⅳ-Ⅴ级、Fisher分级Ⅲ-Ⅳ级、并发急性脑积水患者比例及血清DKK-1、NOX2、sTLT-1水平高于预后良好组(均P<0.05)。Hunt-Hess分级Ⅳ-Ⅴ级、高血清DKK-1、NOX2及sTLT-1水平为aSAH患者介入术后预后不良的独立影响因素(均P<0.05)。血清DKK-1、NOX2、sTLT-1预测aSAH患者介入术后预后不良的曲线下面积(AUC)分别为0.812、0.834、0.798,三者联合预测的AUC提升至0.928,显著高于单一指标(均P<0.05)。结论:aSAH患者血清DKK-1、NOX2、sTLT-1水平升高,与介入治疗后不良预后有关,联合检测对患者预后具有较好的预测效能。
Abstract:
Objective:To investigate the changes and detection significance of serum Dickkopf-1 (DKK-1),nicotinamide adenine dinucleotide phosphate oxidase 2 (NOX2) and soluble triggering receptor expressed on myeloid cells-like transcript-1 (sTLT-1) levels in patients with aneurysmal subarachnoid hemorrhage (aSAH).Methods:Two hundred and thirty aSAH patients who received endovascular intervention were selected as the aSAH group,and another 100 healthy physical examinees were selected as the control group.According to the modified Rankin Scale (mRS) score at 6 months after intervention,patients were divided into good-prognosis group (mRS 0-2,168 cases) and poor-prognosis group (mRS 3-6,62 cases).Serum DKK-1,NOX2 and sTLT-1 levels were detected by ELISA.Multivariate logistic regression was used to analyze the influencing factors of poor prognosis after intervention in aSAH patients.ROC curves were drawn to evaluate the predictive efficacy of each index and their combined detection for poor prognosis after intervention in aSAH patients.Results:Serum DKK-1,NOX2 and sTLT-1 levels in the aSAH group were higher than those in the control group (all P<0.05).The poor-prognosis group exhibited higher proportions of Hunt-Hess grade Ⅳ-Ⅴ,Fisher grade Ⅲ-Ⅳ,acute hydrocephalus,and elevated serum DKK-1,NOX2 and sTLT-1 levels compared with the good-prognosis group (all P<0.05).Hunt-Hess grade Ⅳ-Ⅴ,high serum DKK-1,NOX2 and sTLT-1 levels were independent influencing factors for poor prognosis after intervention in aSAH patients (all P<0.05).The AUC of serum DKK-1,NOX2 and sTLT-1 for predicting poor prognosis after intervention in aSAH patients were 0.812,0.834 and 0.798,respectively.The AUC of the three combined prediction increased to 0.928,which was significantly higher than that of single index (all P<0.05).Conclusion:Serum DKK-1,NOX2 and sTLT-1 levels are elevated in aSAH patients,which are related to poor prognosis after endovascular interventional therapy,and combined detection has good predictive efficacy for patient prognosis.
