摘要:
目的:探讨血清微小RNA-214(miR-214)、Toll样受体4(TLR4)、半乳糖凝集素-3(Galectin-3)与急性缺血性脑卒中(AIS)患者预后的关系。方法:选取128例AIS患者为AIS组,同期116例健康者为对照组。AIS组接受规范治疗后,根据改良Rankin量表(mRS)评估90 d内预后,mRS≤2分为预后良好组(82例),mRS>2分为预后不良组(46例)。比较各组血清miR-214、TLR4、Galectin-3水平;多因素Logistic回归分析AIS患者预后的影响因素;受试者工作特征(ROC)曲线分析血清miR-214、TLR4、Galectin-3对AIS患者预后不良的预测价值。结果:血清miR-214、TLR4、Galectin-3水平比较,AIS组高于对照组,预后不良组高于预后良好组(均P<0.05)。预后不良组梗死体积、发病到住院时间、三酰甘油、同型半胱氨酸(Hcy)及入院时美国国立卫生研究院卒中量表(NIHSS)评分高于预后良好组(均P<0.05)。发病到住院时间长、入院时高NIHSS评分、高血清miR-214水平、高血清TLR4水平、高血清Galectin-3水平是AIS患者预后的独立危险因素(均P<0.05)。血清miR-214、TLR4、Galectin-3单独及联合预测AIS患者预后不良的曲线下面积(AUC)中,联合预测的AUC最高(均P<0.05)。结论:AIS患者血清miR-214、TLR4、Galectin-3水平升高,且均为预后不良的影响因素,三者联合可作为临床评估AIS患者预后的参考指标。
Abstract:
Objective:To investigate the relationship between serum microRNA-214 (miR-214),Toll-like receptor 4 (TLR4),Galectin-3 and prognosis in patients with acute ischemic stroke (AIS).Methods:A total of 128 AIS patients were recruited as the AIS group,and 116 healthy individuals were enrolled as the control group.After receiving standardized treatment,the 90-day prognosis of AIS patients was assessed using the modified Rankin Scale (mRS).Patients with mRS≤2 were classified into the favorable prognosis group (82 cases),while those with mRS>2 were assigned to the poor prognosis group (46 cases).Serum levels of miR-214,TLR4 and Galectin-3 were compared among different groups.Multivariate logistic regression analysis was adopted to identify the factors influencing the prognosis of AIS patients.ROC curve was employed to evaluate the predictive value of serum miR-214,TLR4 and Galectin-3 for poor prognosis in AIS patients.Results:The serum levels of miR-214,TLR4 and Galectin-3 in the AIS group were higher than those in the control group,and the levels in the poor prognosis group were higher than those in the favorable prognosis group (all P<0.05).Compared with the favorable prognosis group,the poor prognosis group had larger infarct volume,longer time from onset to hospitalization,higher levels of triglyceride and homocysteine (Hcy),and higher National Institutes of Health Stroke Scale (NIHSS) score on admission (all P<0.05).Longer time from onset to hospitalization,high NIHSS score on admission,elevated serum miR-214,TLR4 and Galectin-3 levels were independent risk factors for the prognosis of AIS patients (all P<0.05).Among the AUC of serum miR-214,TLR4,Galectin-3 alone and in combination for predicting poor prognosis in AIS patients,the combined prediction showed the highest AUC (all P<0.05).Conclusion:Serum miR-214,TLR4 and Galectin-3 levels are significantly increased in AIS patients,and all of them are associated with poor prognosis.The combination of these three indicators can serve as a reference for the clinical assessment of prognosis in AIS patients.
