摘要:
目的:探讨上皮性卵巢癌(EOC)患者血清腓骨蛋白-1(FBLN-1)、蛋白磷酸酯酶2A(PP2A)、生长刺激表达基因2蛋白(ST2)水平变化及其与预后的关系。方法:选取EOC患者130例为EOC组,另选同期100例健康者为对照组。检测血清FBLN-1、PP2A、ST2水平。随访1年,按生存或复发情况分为预后不良组(42例)和预后良好组(88例)。EOC患者预后的影响因素以多因素Logistic回归进行分析。血清FBLN-1、PP2A、ST2对EOC患者预后的预测价值通过绘制受试者工作特征(ROC)曲线进行分析。结果:EOC组血清FBLN-1、ST2 水平高于对照组,PP2A水平较对照组降低(均P<0.05)。与预后良好组比较,预后不良组血清FBLN-1、ST2水平升高,PP2A水平降低(均P<0.05)。预后不良组国际妇产科联盟(FIGO)分期Ⅲ期、有腹水、肿瘤低分化、有淋巴结转移患者比例高于预后良好组(均P<0.05)。影响EOC患者预后的因素包括血清FBLN-1、PP2A、ST2及FIGO分期、淋巴结转移、腹水、肿瘤分化程度(均P<0.05)。血清FBLN-1、PP2A、ST2联合预测EOC患者预后不良的曲线下面积(AUC)为0.901,较各指标单独预测的AUC高(均P<0.05)。结论:EOC患者血清FBLN-1、ST2水平上升,PP2A水平下降,均为预后不良的影响因素,三者联合对预后具有一定预测效能。
Abstract:
Objective:To investigate the changes in serum levels of fibulin-1 (FBLN-1),protein phosphatase 2A (PP2A),and growth-stimulated expressed gene 2 protein (ST2) in patients with epithelial ovarian cancer (EOC) and their relationship with prognosis.Methods:A total of 130 patients with EOC were enrolled as the EOC group,and 100 healthy individuals were selected as the control group.Serum levels of FBLN-1,PP2A and ST2 were measured.Patients were followed up for one year and divided into a poor prognosis group (42 cases) and a good prognosis group (88 cases) based on survival or recurrence status.Multivariate logistic regression analysis was performed to identify factors influencing the prognosis of EOC patients.The predictive value of serum FBLN-1,PP2A and ST2 for prognosis in EOC patients was evaluated by ROC curve analysis.Results:Serum levels of FBLN-1 and ST2 in the EOC group were higher than those in the control group,while PP2A level was lower (all P<0.05).Compared with the good prognosis group,the poor prognosis group showed elevated serum FBLN-1 and ST2 levels and decreased PP2A level (all P<0.05).The proportions of patients with FIGO stage Ⅲ,ascites,poorly differentiated tumors,and lymph node metastasis in the poor prognosis group were higher than those in the good prognosis group (all P<0.05).Factors affecting the prognosis of EOC patients included serum FBLN-1,PP2A,ST2,FIGO stage,lymph node metastasis,ascites,and tumor differentiation degree (all P<0.05).The AUC of the combined prediction of serum FBLN-1,PP2A and ST2 for poor prognosis in EOC patients was 0.901,which was higher than that of each individual indicator (all P<0.05).Conclusion:Elevated serum FBLN-1 and ST2 levels and decreased PP2A level in EOC patients are risk factors for poor prognosis.The combination of these three biomarkers demonstrates favorable predictive efficacy for prognosis.
