摘要:
目的:探讨膀胱癌患者血清微小RNA-144(miR-144)、miR-618和细胞角蛋白19(CK19)水平变化及检测意义。方法:选取86例膀胱癌患者为膀胱癌组,同期86例泌尿系统良性疾病患者(膀胱炎、前列腺增生、尿路结石)为良性疾病组。采用RT-qPCR检测两组血清miR-144、miR-618水平,ELISA法测定血清CK19浓度,分析三者水平与膀胱癌患者临床病理特征的关系,Pearson法分析血清miR-144、miR-618及CK19水平的相关性,受试者工作特征(ROC)曲线评估各指标及联合检测对膀胱癌与泌尿系统良性疾病的鉴别诊断价值。结果:膀胱癌组血清miR-144水平低于良性疾病组,血清miR-618、CK19水平高于良性疾病组(均P<0.05)。血清miR-144在肿瘤分期T2-T4期、病理分级G3级、有淋巴结转移、肿瘤直径≥3 cm的膀胱癌患者中表达水平低于肿瘤分期Ta-T1期、病理分级G1-G2级、无淋巴结转移、肿瘤直径<3 cm的患者(均P<0.05)。血清miR-618与CK19在肿瘤分期T2-T4期、病理分级G3级、有淋巴结转移、肿瘤直径≥3 cm的膀胱癌患者中表达水平高于肿瘤分期Ta-T1期、病理分级G1-G2级、无淋巴结转移、肿瘤直径<3 cm的患者(均P<0.05)。膀胱癌患者血清miR-144与miR-618、CK19水平呈负相关,血清miR-618与CK19水平呈正相关(均P<0.05)。血清miR-144、miR-618、CK19单独诊断膀胱癌的曲线下面积(AUC)分别为0.753、0.778、0.765,三者联合诊断的AUC为0.897,高于各指标单独诊断的AUC(均P<0.05)。结论:膀胱癌患者血清miR-144呈低表达,miR-618与CK19呈高表达,三者与患者临床病理特征有关,联合检测可提高膀胱癌与泌尿系统良性疾病的鉴别诊断效能。
Abstract:
Objective:To explore the changes of serum microRNA-144 (miR-144),miR-618 and cytokeratin 19 (CK19) levels in patients with bladder cancer and their detection significance.Methods:A total of 86 patients with bladder cancer were enrolled as the bladder cancer group,and 86 patients with benign urinary system diseases (cystitis,benign prostatic hyperplasia,urinary calculi) were selected as the benign disease group.RT-qPCR was used to detect serum levels of miR-144 and miR-618 in both groups,and ELISA was adopted to determine serum CK19 concentration.The relationship between the three indicators and the clinicopathological features of bladder cancer patients was analyzed.Pearson correlation analysis was used to explore the correlation among serum miR-144,miR-618 and CK19 levels.ROC curve was employed to evaluate the differential diagnostic value of each indicator alone and in combination for bladder cancer and benign urinary system diseases.Results:Serum miR-144 level in the bladder cancer group was lower than that in the benign disease group,while serum miR-618 and CK19 levels were higher than those in the benign disease group (all P<0.05).Serum miR-144 expression level was lower in bladder cancer patients with tumor stage T2-T4,pathological grade G3,lymph node metastasis and tumor diameter ≥3 cm than in those with tumor stage Ta-T1,pathological grade G1-G2,no lymph node metastasis and tumor diameter <3 cm (all P<0.05).Serum miR-618 and CK19 expression levels were higher in bladder cancer patients with tumor stage T2-T4,pathological grade G3,lymph node metastasis and tumor diameter ≥3 cm than in those with tumor stage Ta-T1,pathological grade G1-G2,no lymph node metastasis and tumor diameter <3 cm (all P<0.05).In bladder cancer patients,serum miR-144 level was negatively correlated with miR-618 and CK19 levels,and serum miR-618 level was positively correlated with CK19 level (all P<0.05).The AUC of serum miR-144,miR-618 and CK19 alone for diagnosing bladder cancer was 0.753,0.778 and 0.765,respectively.The AUC of the combined detection of the three indicators was 0.897,which was higher than that of each indicator alone (all P<0.05).Conclusion:Serum miR-144 is lowly expressed,while miR-618 and CK19 are highly expressed in patients with bladder cancer.The three indicators are related to the clinicopathological features of patients,and combined detection can improve the differential diagnostic efficacy between bladder cancer and benign urinary system diseases.
