摘要:
目的:探讨外周血富半胱氨酸蛋白61(CYR61)、孤独G蛋白偶联受体配体13(Apelin-13)水平及扩散张量成像(DTI)扫描参数与急性脑梗死(ACI)患者早期神经功能恶化及预后的关系。方法:选取治疗的ACI患者116例,比较是否发生早期神经功能恶化、预后不良和良好患者临床资料及外周血CYR61、Apelin-13水平和DTI扫描参数差异。结果:发生早期神经功能恶化患者年龄、入院时美国国立卫生研究院卒中量表(NIHSS)评分、CYR61和衰减指数(EXAT)分别为(68.83±8.22)岁、(10.50±2.10)分、(2786.54±223.32)pg/ml和(0.53±0.17)×10-4 mm2/s,高于未发生早期神经功能恶化患者(均P<0.05),而Apelin-13、异性指数(FA)和平均弥散系数(DCAVG)分别为(0.82±0.21)ng/ml、(0.22±0.08)×10-4 mm2/s和(1.71±0.34)×10-4  mm2/s,低于未发生早期神经功能恶化患者(均P<0.05)。Logistic回归分析显示患者发生早期神经功能恶化的影响因素包括:入院时NIHSS评分、CYR61、Apelin-13(均P<0.05)。年龄、入院时NIHSS评分、CYR61和EXAT比较,预后不良患者分别为(70.02±8.12)岁、(9.92±1.82)分、(2803.32±210.05)pg/ml和(0.52±0.14)×10-4  mm2/s,高于预后良好患者(均P<0.05),而Apelin-13、FA和DCAVG分别为(0.78±0.19)ng/ml、(0.21±0.06)×10-4 mm2/s和(1.66±0.31)×10-4 mm2/s,低于预后良好患者(均P<0.05)。Logistic回归分析显示预后不良的影响因素包括:入院时NIHSS评分、CYR61、Apelin-13、EXAT、FA和DCAVG(均P<0.05)。结论:外周血CYR61、Apelin-13水平是ACI患者早期神经功能恶化的影响因素,同时CYR61、Apelin-13水平及DTI扫描参数是ACI患者预后的影响因素。
Abstract:
Objective:To explore the relationship between peripheral blood levels of cysteine rich protein 61 (CYR61),isolated G protein coupled receptor ligand 13 (Apelin-13),diffusion tensor imaging (DTI) scan parameters,and early neurological deterioration and prognosis in patients with acute cerebral infarction (ACI).Methods:116 patients with ACI treated from January 2021 to March 2024 were selected for followup observation of early neurological deterioration and prognosis.The clinical data of patients with and without early neurological deterioration,poor prognosis,and good outcomes were compared.Results:The age,National Institutes of Health Stroke Scale (NIHSS) score,CYR61,and Attenuation Index (EXAT) of patients with early neurological deterioration were (68.83±8.22) years,(10.50±2.10) points,(2786.54±223.32) pg/ml,and (0.53±0.17)×10-4  mm2/s,respectively,which were higher than those of patients without early neurological deterioration (all P<0.05),the Apelin-13,Heterogeneity Index (FA),and Mean Diffusion Coefficient (DCAVG) were (0.82±0.21) ng/ml,(0.22±0.08)×10-4  mm2/s,and (1.71±0.34) × 10-4  mm2/s,respectively,which were lower than those of patients without early neurological deterioration (all P<0.05).Logistic regression analysis showed that NIHSS score at admission,CYR61,and Apelin-13 were influencing factors for early deterioration of neurological function in patients (all P<005).The age,NIHSS score at admission,CYR61,and EXAT of patients with poor prognosis were (70.02±8.12) years,(9.92±1.82) points,(2803.32±210.05)pg/ml,and (0.52±0.14)×10-4  mm2/s,respectively,which were higher than those of patients with good prognosis (all P<0.05),the Apelin-13,FA,and DCAVG were (0.78±0.19) ng/ml,(0.21±0.06)×10-4 mm2/s,and (1.66±0.31)×10-4  mm2/s s,respectively,which were lower than those of patients with good prognosis (all P<0.05).Logistic regression analysis showed that NIHSS score at admission,CYR61,Apelin-13,EXAT,FA,and DCAVG were factors affecting poor prognosis in patients (all P<0.05).Conclusion:The levels of CYR61 and Apelin-13 in peripheral blood are influencing factors for early deterioration of neurological function in ACI patients,while the levels of CYR61 and Apelin-13 and DTI scan parameters are influencing factors for the prognosis of ACI patients.
