摘要:
目的:探讨肺炎支原体肺炎(MPP)患儿血清长链非编码RNA核富集转录体1(lncRNA NEAT1)、微小RNA-128-3p(miR-128-3p)水平与病情严重程度的关系,并分析两者对预后的预测价值。方法:选取MPP患儿152例为MPP患儿组,选取同期健康体检儿童150例为健康儿童组。MPP患儿入院时根据病情程度分为MPP轻症组(84例)和MPP重症组(68例),又依据治疗30 d后预后分为MPP预后良好组和MPP预后不良组。比较健康儿童组与MPP患儿组、MPP轻症组与MPP重症组血清lncRNA NEAT1、miR-128-3p水平;比较不同预后MPP患儿临床资料、实验室指标及血清lncRNA NEAT1、miR-128-3p水平;Pearson法分析MPP患儿血清lncRNA NEAT1、miR-128-3p与临床资料、实验室指标的相关性;Logistic回归分析预后影响因素;受试者工作特征(ROC)曲线评价两者对预后不良的预测价值。结果:MPP患儿组血清lncRNA NEAT1、miR-128-3p显著高于健康儿童组(均P<0.05)。MPP重症组血清lncRNA NEAT1、miR-128-3p显著高于轻症组(均P<0.05)。治疗30 d后,预后不良患儿30例,预后良好患儿122例。与MPP预后良好组比较,预后不良组发热持续时间、体温峰值、中性粒细胞百分比(NEUT%)、乳酸脱氢酶(LDH)、C-反应蛋白(CRP)、D-二聚体(D-D)、lncRNA NEAT1、miR-128-3p水平显著升高(均P<0.05)。MPP患儿血清lncRNA NEAT1与miR-128-3p呈正相关,且两者与NEUT%、LDH、CRP、D-D呈正相关(均P<0.05)。发热持续时间长及NEUT%、LDH、CRP、D-D、lncRNA NEAT1、miR-128-3p水平高是MPP患儿预后的独立危险因素(均P<0.05)。血清lncRNA NEAT1、miR-128-3p联合预测MPP患儿出现预后不良的曲线下面积(AUC)大于lncRNA NEAT1、miR-128-3p单项(均P<0.05)。结论:MPP患儿血清lncRNA NEAT1、miR-128-3p均呈现高水平,且病情越重两者水平越高。两项联合对MPP患儿出现预后不良的预测价值较好。
Abstract:
Objective:To explore the relationship between the levels of serum long non-coding RNA nuclear enriched abundant transcript 1 (lncRNA NEAT1),microRNA-128-3p (miR-128-3p) in the children with mycoplasma pneumoniae pneumonia (MPP) and the severity of the disease,and to analyze the predictive value of both for prognosis.Methods:A total of 152 MPP children and 150 healthy children with physical examination during the same period were selected as MPP children group and healthy children group,respectively.Upon admission,the children with MPP were divided into MPP mild group (84 cases) and MPP severe group (68 cases) according to the severity of their conditions.After 30 days of treatment,the children with MPP were divided into MPP good prognosis group and MPP poor prognosis group according to the prognosis.The levels of serum lncRNA NEAT1 and miR-128-3p between healthy children group and MPP children group,MPP mild group and MPP severe group were compared.The clinical data,laboratory indicators and serum lncRNA NEAT1,miR-128-3p levels of MPP children with different prognosis were compared.The correlation of serum lncRNA NEAT1 and miR-128-3p with clinical data,laboratory indicators in MPP children was analyzed by Pearson method.Logistic regression analysis was performed to analyze the factors affecting the prognosis of MPP children.ROC curve was used to evaluate the predictive value of serum lncRNA NEAT1 and miR-128-3p for poor prognosis in MPP children.Results:MPP children group had higher serum lncRNA NEAT1 and miR-128-3p levels compared with healthy children group (all P<0.05).MPP severe group had higher serum lncRNA NEAT1 and miR-128-3p compared with mild group (all P<0.05).After 30 days of treatment,there were 30 cases of poor prognosis and 122 cases of good prognosis among the children.Compared with MPP good prognosis group,the fever duration,peak body temperature,NEUT%,LDH,CRP,D-D,lncRNA NEAT1 and miR-128-3p levels in poor prognosis group were significantly increased (all P<0.05).Serum lncRNA NEAT1 and miR-128-3p were positively correlated in MPP children,and both were positively correlated with NEUT%,LDH,CRP,and D-D (all P<0.05).Long duration of fever,high levels of NEUT%,LDH,CRP,D-D,lncRNA NEAT1 and miR-128-3p were all independent risk factors for prognosis in MPP children (all P<0.05).The AUC of serum lncRNA NEAT1 and miR-128-3p combined to predict poor prognosis in MPP children was larger than that of lncRNA NEAT1 and miR-128-3p alone (all P<0.05).Conclusion:The serum lncRNA NEAT1 and miR-128-3p in MPP children are both at high levels,and the more severe the disease,the higher the levels of both.The combination of lncRNA NEAT1 and miR-128-3p had better predictive value for poor prognosis in MPP children.
