目的:探讨尖锐湿疣(CA)皮损组织中第二个线粒体衍生半胱天冬氨酸蛋白酶激活剂(Smac)、缺氧诱导因子-1α(HIF-1α)、自噬相关基因5(ATG5)表达与患者临床病理特征和复发的关系。方法:选取98例CA患者为CA组,选同期非皮肤病、因其他原因切除外生殖器正常皮肤组织的志愿者85例为对照组。免疫组织化学染色法检测组织中Smac、HIF-1α、ATG5蛋白表达。随访6个月,记录复发情况。以四格表法分析皮损组织Smac、HIF-1α、ATG5蛋白表达对CA患者复发的预测价值。结果:与对照组正常皮肤组织比较,CA组皮损组织中Smac、ATG5蛋白阳性表达率降低,HIF-1α阳性表达率升高(均P<0.05)。不同病程CA患者Smac、HIF-1α、ATG5蛋白阳性表达率比较差异无统计学意义(均P>0.05)。不同病变范围、疣体数目、尿道受累、HPV感染类型患者皮损组织Smac、HIF-1α、ATG5蛋白阳性表达率比较差异有统计学意义(均P<0.05)。随访6个月,98例CA患者复发40例,未复发58例。Smac蛋白阳性患者复发率10.53%低于Smac阴性患者48.10%,HIF-1α蛋白阳性患者复发率87.65%高于HIF-1α阴性患者11.76%,ATG5蛋白阳性患者复发率11.54%低于ATG5阴性患者50.00%(均P<0.05)。皮损组织Smac、HIF-1α、ATG5蛋白表达联合预测CA患者复发的敏感度92.50%、特异度91.38%、阳性预测值88.10%、阴性预测值91.38%、准确率91.84%,高于单独指标预测(均P<0.05)。结论:Smac、HIF-1α、ATG5在CA皮损组织中异常表达,与临床病理特征及复发风险有关,三者联合对CA患者复发具有一定预测价值。
Abstract:
Objective:To investigate the expression of second mitochondria-derived activator of caspase (Smac),hypoxia inducible factor-1α(HIF-1α),and sutophagy-related gene 5 (ATG5) in condyloma acuminatum (CA) lesions and their relationship with clinicopathological characteristics and recurrence.Methods:A total of 98 CA patients were enrolled as the CA group,and 85 volunteers who underwent excision of normal external genital skin tissue for non-dermatological reasons during the same period served as the control group.Immunohistochemical staining was used to detect the protein expression of Smac,HIF-1α and ATG5 in tissue samples.Patients were followed up for 6 months to record recurrence.A 2×2 contingency table was used to analyze the predictive value of Smac,HIF-1α and ATG5 protein expression for recurrence in CA patients.Results:Compared with normal skin tissue in the control group,the positive expression rates of Smac and ATG5 proteins were significantly lower,while the positive expression rate of HIF-1α was significantly higher in CA lesion tissues (all P<0.05).There was no significant difference in the positive expression rates of Smac,HIF-1α,and ATG5 among CA patients with different disease durations (all P>0.05).However,significant differences were observed in the positive expression rates of these proteins among patients with different lesion extents,numbers of warts,urethral involvement,and HPV infection types (all P<0.05).During the 6-month follow-up,40 of 98 CA patients experienced recurrence,while 58 did not.The recurrence rate in Smac-positive patients (10.53%) was significantly lower than in Smac-negative patients (48.10%).The recurrence rate in HIF-1α-positive patients (87.65%) was significantly higher than in HIF-1α-negative patients (11.76%).The recurrence rate in ATG5-positive patients (11.54%) was significantly lower than in ATG5-negative patients (50.00%) (all P<0.05).The combined prediction model based on Smac,HIF-1α,and ATG5 expression achieved a sensitivity of 92.50%,specificity of 91.38%,positive predictive value of 88.10%,negative predictive value of 91.38%,and accuracy of 91.84%,which were significantly higher than those of individual predictors (all P<0.05).Conclusion:Abnormal expression of Smac,HIF-1α,and ATG5 in CA lesion tissues is associated with clinicopathological characteristics and recurrence risk.The combined detection of these three markers holds predictive value for recurrence in CA patients.
