摘要:
目的:探讨口腔癌(OC)患者血清三叶因子1(TFF1)、细胞角蛋白19片段抗原21-1(CYFRA21-1)、尿激酶型纤溶酶原激活剂(uPA)水平变化及检测意义。方法:选取OC患者129例为OC组,根据患者3年生存情况分为生存组(85例)和病死组(44例),另选同期体检健康者100例为对照组。比较各组血清TFF1、CYFRA21-1、uPA水平。以多因素Logistic回归分析OC患者预后的影响因素;采用Spearman法分析血清TFF1、CYFRA21-1、uPA水平与OC患者死亡结局的相关性;通过受试者工作特征(ROC)曲线分析血清TFF1、CYFRA21-1、uPA对OC患者死亡结局的预测价值。结果:与对照组比较,OC组患者血清TFF1、CYFRA21-1、uPA水平升高(均P<0.05)。与生存组比较,病死组患者血清TFF1、CYFRA21-1、uPA水平升高(均P<0.05)。病死组有吸烟史、淋巴结转移、TNM分期Ⅲ-Ⅳ期、肿瘤低分化/未分化患者比例以及肿瘤浸润深度高于生存组(均P<0.05)。有吸烟史、淋巴结转移、TNM分期Ⅲ-Ⅳ期、肿瘤低/未分化以及高血清TFF1、CYFRA21-1、uPA水平为OC患者预后的危险因素(均P<0.05)。血清TFF1、CYFRA21-1、uPA水平与OC患者死亡结局呈正相关(均P<0.05)。血清TFF1、CYFRA21-1、uPA联合预测OC患者死亡结局的曲线下面积(AUC)值为0.911,高于三者单独预测的AUC(均P<0.05)。结论:OC患者血清TFF1、CYFRA21-1、uPA水平升高,三者与患者预后有关,联合检测对预后有一定预测价值。
Abstract:
Objective:To investigate the changes and detection significance of serum trefoil factor 1 (TFF1),cytokeratin 19 fragment antigen 21-1 (CYFRA21-1),and urokinase-type plasminogen activator (uPA) levels in patients with oral cancer (OC).Methods:A total of 129 OC patients were enrolled as the OC group and further divided into a survival group (85 cases) and a death group (44 cases) based on 3-year survival outcomes.Additionally,100 healthy individuals were selected as the control group.Serum levels of TFF1,CYFRA21-1 and uPA were compared among the groups.Multivariate logistic regression was used to analyze prognostic factors in OC patients.Spearman correlation analysis was applied to assess the relationship between serum TFF1,CYFRA21-1 and uPA levels and death outcomes.ROC curves were constructed to evaluate the predictive value of these biomarkers for death in OC patients.Results:Compared with the control group,serum levels of TFF1,CYFRA21-1 and uPA were significantly elevated in the OC group (all P<0.05).Compared with the survival group,the death group exhibited higher serum levels of TFF1,CYFRA21-1 and uPA (all P<0.05).The death group also had higher proportions of patients with a history of smoking,lymph node metastasis,TNM stage Ⅲ-Ⅳ,poorly differentiated/undifferentiated tumors,and greater tumor invasion depth (all P<0.05).Multivariate analysis identified a history of smoking,lymph node metastasis,TNM stage Ⅲ-Ⅳ,poor/undifferentiated tumor grade,and elevated serum levels of TFF1,CYFRA21-1,and uPA as independent risk factors for poor prognosis in OC patients (all P<0.05).Serum TFF1,CYFRA21-1,and uPA levels were positively correlated with death outcomes (all P<0.05).The AUC for the combined prediction of death using TFF1,CYFRA21-1 and uPA was 0.911,which was significantly higher than that of any single biomarker (all P<0.05).Conclusion:Serum levels of TFF1,CYFRA21-1,and uPA are elevated in OC patients and are associated with prognosis.Combined detection of these biomarkers holds predictive value for clinical outcomes.
