摘要:
目的：探讨子宫内膜癌（EC）患者血清表皮生长因子受体（EGFR）、人表皮生长因子受体2（HER2）及糖类抗原125（CA125）水平变化及检测意义。方法：选取130例EC患者作为EC组，同期体检健康女性90例作为对照组。随访18个月，根据有无复发及生存情况分为预后良好组（94例）和预后不良组（36例）。采用ELISA法检测血清EGFR、HER2、CA125水平。Logistic回归分析患者预后的危险因素，血清EGFR、HER2、CA125单独及联合检测对EC患者预后不良的预测价值以受试者工作特征（ROC）曲线分析，Kaplan-Meier曲线分析血清EGFR、HER2、CA125水平与EC患者复发的关系。结果：EC组血清EGFR、HER2、CA125水平高于对照组（均P<0.05）。不同肌层浸润深度、肿瘤分化程度、淋巴结转移、国际妇产科联盟（FIGO）分期患者血清EGFR、HER2、CA125水平差异有统计学意义（均P<0.05）。预后不良组中有肿瘤低分化、淋巴结转移、FIGO分期Ⅲ-Ⅳ期、肌层浸润深度≥1/2肌层的患者比例更高，且血清EGFR、HER2、CA125水平高于预后良好组（均P<0.05）。肿瘤低分化、FIGO分期Ⅲ-Ⅳ期及血清EGFR、HER2、CA125水平升高是EC患者预后的独立危险因素（均P<0.05）。血清EGFR、HER2、CA125联合预测EC患者预后不良的曲线下面积（AUC）为0.883，高于单独检测（均P<0.05）。高浓度EGFR、HER2、CA125患者平均复发时间短于低浓度患者（均P<0.05）。结论：EC患者血清EGFR、HER2、CA125水平升高，与临床病理特征及预后有关，三者联合对预后有较高的预测价值。
Abstract:
Objective:To investigate the changes and detection significance of serum epidermal growth factor receptor (EGFR),human epidermal growth factor receptor 2 (HER2) and carbohydrate antigen 125 (CA125) levels in patients with endometrial cancer (EC).Methods：One hundred and thirty EC patients were selected as the EC group,and 90 healthy women were taken as the control group.After 18 months of follow-up,according to recurrence and survival status,they were divided into a good-prognosis group (94 cases) and a poor-prognosis group (36 cases).Serum EGFR,HER2 and CA125 levels were detected by ELISA.Logistic regression was used to analyze the risk factors for patient prognosis.The predictive value of serum EGFR,HER2 and CA125 alone and in combination for poor prognosis in EC patients was analyzed by ROC curves.Kaplan-Meier curves were used to analyze the relationship between serum EGFR,HER2 and CA125 levels and recurrence in EC patients.Results:Serum EGFR,HER2 and CA125 levels in the EC group were higher than those in the control group (all P<0.05).Differences in serum EGFR,HER2 and CA125 levels among patients with different myometrial invasion depths,tumor differentiation grades,lymph-node metastasis status and FIGO stages were statistically significant (all P<0.05).The proportions of patients with poorly differentiated tumors,lymph-node metastasis,FIGO stage Ⅲ-Ⅳ and myometrial invasion depth≥1/2 myometrium in the poor-prognosis group were higher,and serum EGFR,HER2 and CA125 levels were higher than those in the good-prognosis group (all P<0.05).Poor tumor differentiation,FIGO stage Ⅲ-Ⅳ and elevated serum EGFR,HER2 and CA125 levels were independent risk factors for prognosis in EC patients (all P<0.05).The AUC of serum EGFR,HER2 and CA125 combined for predicting poor prognosis in EC patients was 0.883,which was higher than that of each single test (all P<0.05).The mean recurrence time of patients with high concentrations of EGFR,HER2 and CA125 was shorter than that of patients with low concentrations (all P<0.05).Conclusion:Serum EGFR,HER2 and CA125 levels are elevated in EC patients,are related to clinicopathological features and prognosis,and their combination has a high predictive value for prognosis.
