摘要:
目的:探讨钼靶X射线联合血清组织多肽特异性抗原(TPS)、乳脂球表皮生长因子8(MFG-E8)对乳腺癌(BC)的诊断及预后预测价值。方法:选取105例乳腺肿块患者,根据病理检查结果将其分为良性病变组(40例)和BC组(65例),同时选取同期健康女性80例为对照组。以Kappa检验分析钼靶X射线检查与病理诊断结果的一致性。比较三组血清TPS、MFG-E8水平。随访1年,记录患者死亡、复发或转移情况,并分为预后良好组(53例)和预后不良组(12例)。比较不同预后BC患者钼靶X射线恶性特征及血清TPS、MFG-E8水平。绘制受试者工作特征(ROC)曲线分析钼靶X射线检查、血清TPS及MFG-E8对BC的诊断及预后预测价值。结果:钼靶X射线检查结果显示,105例乳腺肿块患者中,乳腺恶性病变63例,良性病变42例。钼靶X射线检查与病理诊断结果的Kappa值为0.642(P<0.05)。BC组血清TPS、MFG-E8水平高于良性病变组与对照组(均P<0.05)。钼靶X射线检查、血清TPS及MFG-E8联合诊断BC的敏感度、特异度、阳性预测值、阴性预测值、准确率高于三项指标单独诊断(均P<0.05)。预后不良组钼靶X射线显示恶性特征比例及血清TPS、MFG-E8水平高于预后良好组(均P<0.05)。钼靶X射线检查、血清TPS及MFG-E8联合预测BC患者预后的敏感度、特异度、阳性预测值、阴性预测值、准确率高于三项指标单独预测(均P<0.05)。结论:钼靶X射线检查联合血清TPS、MFG-E8对BC有一定的诊断及预后预测价值。
Abstract:
Objective:To evaluate the diagnostic and prognostic predictive value of mammographic X-ray combined with serum tissue polypeptide specific antigen (TPS) and milk fat globule epidermal growth factor 8 (MFG-E8) in breast cancer (BC).Methods:A total of 105 patients with breast masses were enrolled and divided into a benign lesion group (40 cases) and a BC group (65 cases) based on pathological findings.Eighty healthy women served as the control group.Concordance between mammographic X-ray and pathological diagnosis was assessed using the Kappa test.Serum levels of TPS and MFG-E8 were compared across the three groups.Patients were followed up for 1 year,and outcomes were recorded.Based on the occurrence of death,recurrence,or metastasis,BC patients were further classified into a favorable prognosis group (53 cases) and a poor prognosis group (12 cases).Malignant mammographic features and serum TPS and MFG-E8 levels were compared between prognostic subgroups.The ROC curve was drawn to analyze the diagnostic and prognostic predictive value of mammography X-ray examination,serum TPS and MFG-E8 for BC.Results:Mammographic X-ray identified 63 malignant and 42 benign lesions among the 105 breast mass patients.The Kappa value for agreement between mammographic and pathological findings was 0.642 (P<0.05).Serum TPS and MFG-E8 levels in the BC group were significantly higher than those in the benign lesion and control groups (all P<0.05).The combined use of mammographic X-ray,serum TPS,and MFG-E8 yielded superior sensitivity,specificity,positive predictive value,negative predictive value,and overall accuracy in diagnosing BC compared with each modality alone (all P<0.05).The proportion of malignant features shown by mammography X-ray and the levels of serum TPS and MFG-E8 in the poor prognosis group were higher than those in the good prognosis group (all P<0.05).The combined prediction of mammography X-ray examination,serum TPS and MFG-E8 for the prognosis of BC patients had higher sensitivity,specificity,positive predictive value,negative predictive value and accuracy than the prediction of any one of the three indicators alone (all P<0.05).Conclusion:The combination of mammographic X-ray with serum TPS and MFG-E8 offers valuable diagnostic and prognostic information in BC.
