摘要:
目的:探讨数字化导板在美学区即刻种植中的应用效果。方法:选取行美学区即刻种植患者43例,根据不同手术方式分为观察组(22例,29颗患牙)和对照组(21例,27颗患牙)。对照组行传统自由手即刻种植,观察组行数字化导板辅助即刻种植。比较两组术后种植体植入精确度(角度偏差、颈部距离偏差、根尖距离偏差、深度偏差)。比较两组修复后即刻及3个月美学效果[红色美学评分(PES)、白色美学评分(WES)、牙龈乳头指数(PIS)]。比较两组术前及修复后3个月牙周健康指标[菌斑指数(PLI)、改良龈沟出血指数(mSBI)、牙龈指数(GI)]。比较两组患者满意度。结果:观察组角度偏差、颈部距离偏差、根尖距离偏差及深度偏差小于对照组(均P<0.05)。修复后即刻,观察组PES、WES评分高于对照组(均P<0.05)。修复后3个月,两组PES、WES、PIS评分较修复后即刻升高,且观察组PES、WES、PIS评分高于对照组(均P<0.05)。治疗后,两组PLI、mSBI、GI指标较治疗前改善,且观察组mSBI、GI指标改善效果优于对照组(均P<0.05)。观察组患者修复后总满意度较高于对照组,但差异无统计学意义(P>0.05)。结论:数字化导板辅助下美学区即刻种植能够提升种植体植入位点精确度,改善美学效果及患牙牙周健康,提高患者满意度。
Abstract:
Objective:To investigate the clinical efficacy of digital surgical guides for immediate implantation in the aesthetic zone.Methods:A total of 43 patients requiring immediate implantation in the aesthetic zone were selected and divided into an observation group (22 patients,29 teeth) and a control group (21 patients,27 teeth) based on different surgical protocols.The control group underwent conventional free-hand immediate implantation,while the observation group received computer-guided template-assisted immediate implantation.The postoperative implant placement accuracy (angular deviation,neck deviation,apex deviation,depth deviation) was compared between the two groups.Aesthetic outcomes were assessed immediately and 3 months post-restoration using the Pink Esthetic Score (PES),White Esthetic Score (WES),and Papilla Index Score (PIS).Periodontal health indicators,including the Plaque Index (PLI),modified Sulcus Bleeding Index (mSBI),and Gingival Index (GI),were compared before and 3 months post-restoration.Patient satisfaction was also evaluated.Results:The observation group showed significantly smaller angular deviation,neck deviation,apex deviation,and depth deviation compared to the control group (all P<0.05).Immediately post-restoration,the PES and WES scores in the observation group were higher than those in the control group (all P<0.05).At 3 months post-restoration,the PES,WES,and PIS scores in both groups increased from the immediate post-restoration levels,with the observation group demonstrating superior scores (all P<0.05).After treatment,both groups showed improvement in PLI,mSBI,and GI compared to baseline,and the observation group exhibited better improvement in mSBI and GI than the control group (all P<0.05).The overall post-restoration satisfaction rate was higher in the observation group,although the difference was not statistically significant (P>0.05).Conclusion:Computer-guided template-assisted immediate implantation in the aesthetic zone can enhance the precision of implant placement,improve aesthetic outcomes and periodontal health of the treated teeth,and yield higher patient satisfaction.
