摘要:
肾纤维化是多种慢性肾脏疾病的常见病理过程,表现为肾小管-间质损伤、炎症反应、成纤维细胞活化和细胞外基质过度积累等。随着病程的进展,肾功能逐渐丧失,最终可能导致终末期肾脏病。内源性大麻素系统(ECS)由大麻素受体、内源性配体及合成/降解酶组成,广泛参与炎症、氧化应激和纤维化的调控。近年来研究发现,ECS在肾纤维化中具有双重作用:CB1R激活可能促进纤维化,而CB2R激活则可能发挥抗纤维化效应。因此,深入探讨ECS在肾纤维化中的作用,将为肾纤维化的早期诊断和治疗提供新的理论依据和临床策略。
Abstract:
Renal fibrosis is a common pathological process in various chronic kidney diseases,characterized by tubulointerstitial damage,inflammatory responses,fibroblast activation,and excessive accumulation of extracellular matrix.As the disease progresses,renal function progressively deteriorates,which may ultimately lead to end-stage renal disease.The endocannabinoid system (ECS),comprising cannabinoid receptors,endogenous ligands,and synthetic/degradative enzymes,is widely involved in regulating inflammation,oxidative stress,and fibrotic processes.Recent research has revealed a dual role of the ECS in renal fibrosis:activation of cannabinoid receptor type 1 (CB1R) may promote fibrogenesis,whereas activation of cannabinoid receptor type 2 (CB2R) likely exerts anti-fibrotic effects.Therefore,in-depth investigation into the role of the ECS in renal fibrosis will provide novel theoretical foundations and clinical strategies for early diagnosis and treatment of this condition.
