目的:探讨血清趋化素样因子1(CKLF1)、18 kDa转运蛋白(TSPO)、可溶性肿瘤坏死因子样凋亡弱诱导因子(sTWEAK)检测对重型创伤性颅脑损伤(sTBI)患者预后不良的预测价值。方法:选取128例sTBI患者,随访1年,以格拉斯哥预后量表(GOS)评分将患者分为预后良好组(75例)和预后不良组(53例)。比较两组血清CKLF1、TSPO、sTWEAK水平。多因素Logistic回归分析sTBI患者预后不良的影响因素。受试者工作特征曲线(ROC)分析血清CKLF1、TSPO、sTWEAK对sTBI患者预后不良的预测。结果:血清CKLF1、TSPO、sTWEAK水平比较,预后不良组较预后良好组升高(均P<0.05)。与预后良好组比较,预后不良组入院时格拉斯哥昏迷评分(GCS)、白蛋白(ALB)水平降低,入院时急性生理学与慢性健康状况评分系统Ⅱ(APACHEⅡ)、血清肌酐(Scr)水平升高,且患者机械通气、多发脑挫裂伤比例较高(均P<0.05)。多发脑挫裂伤、机械通气、血清CKLF1、TSPO、sTWEAK及入院时GCS评分、APACHEⅡ评分、Scr、ALB为sTBI患者预后不良的独立影响因素(均P<0.05)。血清CKLF1、TSPO、sTWEAK联合预测sTBI患者预后不良的曲线下面积(AUC)高于三者单独预测的AUC(均P<0.05)。结论:sTBI预后不良患者血清CKLF1、TSPO、sTWEAK水平升高,且三者联合检测对患者预后不良具有一定预测价值。
Abstract:
Objective:To investigate the predictive value of serum chemokine-like factor 1 (CKLF1),18 kDa translocator protein (TSPO),and soluble tumor necrosis factor-like weak inducer of apoptosis (sTWEAK) detection for poor prognosis in patients with severe traumatic brain injury (sTBI).Methods:A total of 128 sTBI patients were followed up for 1 year and divided into good prognosis (75 cases) and poor prognosis (53 cases) groups based on the Glasgow Outcome Scale (GOS) score.Serum CKLF1,TSPO and sTWEAK levels were compared between the groups.Multivariate logistic regression analysis was performed to identify the independent risk factors for poor prognosis in sTBI patients.The predictive value of serum CKLF1,TSPO and sTWEAK for poor prognosis in sTBI patients was analyzed using the ROC curve.Results:Serum CKLF1,TSPO and sTWEAK levels were higher in the poor prognosis group (all P<0.05).Compared with the good prognosis group,the poor prognosis group had lower Glasgow Coma Scale (GCS) and albumin (ALB) levels at admission,higher APACHEⅡ and serum creatinine (Scr) levels,and a higher proportion of mechanical ventilation and multiple cerebral contusions (all P<0.05).Multiple cerebral contusions,mechanical ventilation,serum CKLF1,TSPO,sTWEAK,GCS score,APACHEⅡ score,Scr,and ALB at admission were identified as independent risk factors for poor prognosis in sTBI patients (all P<0.05).The AUC for the combined prediction of poor prognosis in sTBI patients using serum CKLF1,TSPO and sTWEAK was higher than that for individual prediction (all P<0.05).Conclusion:Elevated levels of serum CKLF1,TSPO and sTWEAK are associated with poor prognosis in sTBI patients,and combined detection of these markers has predictive value for poor prognosis.
