摘要:
睡眠剥夺是因环境、行为或病理因素导致的睡眠时长不足或质量下降,从而引发生理和心理状态损伤的现象。随着社会发展,电子设备依赖,工作压力等使人们睡眠时间越来越少,睡眠剥夺成为全球普遍现象。银屑病是以皮肤红斑、鳞屑和瘙痒为主要临床表现的慢性炎症免疫性疾病,基本病理特征为角质形成细胞的异常增殖和分化及免疫细胞过度浸润。银屑病患者常伴随睡眠结构异常,而睡眠剥夺可能通过多种机制进一步加重银屑病的进展。研究表明,睡眠剥夺能够显著提升肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)等炎症因子表达,激活免疫系统。此外,睡眠剥夺扰乱生物钟,影响皮肤细胞的代谢与修复功能,导致皮肤屏障受损。同时,睡眠剥夺引发的氧化应激和胰岛素抵抗等代谢异常也与银屑病的病情恶化密切相关。现对睡眠剥夺加重银屑病的机制及相关治疗策略进行综述,以期为银屑病相关机制研究及干预措施提供参考。
Abstract:
Sleep deprivation,defined as insufficient sleep duration or diminished sleep quality due to environmental,behavioral,or pathological factors,impairs both physiological and psychological functions.In modern society,increasing reliance on electronic devices and rising work-related stress have reduced overall sleep time,making sleep deprivation a widespread problem.Psoriasis is a chronic,immune-mediated inflammatory disease characterized clinically by erythema,scaling,and pruritus.Its core pathology involves abnormal keratinocyte proliferation and differentiation alongside excessive immune-cell infiltration.Psoriatic patients frequently exhibit disrupted sleep architecture,and sleep deprivation may further worsen psoriasis through multiple pathways.Evidence shows that sleep loss upregulates proinflammatory cytokines,such as tumor necrosis factor-α (TNF-α) and Interleukin-6 (IL-6),thereby potentiating immune activation.Moreover,sleep deprivation disturbs circadian rhythms,impairing skin-cell metabolism and repair processes and compromising barrier function.Metabolic derangements induced by insufficient sleep,including oxidative stress and insulin resistance,also correlate closely with psoriasis exacerbation.This article reviews the mechanisms by which sleep deprivation aggravates psoriasis and discusses potential therapeutic strategies,providing a reference framework for future mechanistic studies and interventions.
