摘要:
目的:探究创伤性颅脑损伤(TBI)患者血清补体3a受体1(C3AR1)、血管生成素Ⅱ(AngⅡ)、血管内皮生长因子(VEGF)水平变化及检测意义。方法:选取88例TBI患者,根据格拉斯哥昏迷量表(GCS)将TBI患者分为轻度组(28例)、中度组(33例)和重度组(27例)。根据是否发生急性创伤性凝血病(ATC)将TBI患者分为ATC组(41例)和非ATC组(47例)。采用ELISA法检测血清C3AR1、AngⅡ、VEGF水平。Spearman法分析TBI患者血清C3AR1、AngⅡ、VEGF水平与GCS评分的相关性。采用Logistic回归分析TBI患者并发ATC的影响因素。血清C3AR1、AngⅡ、VEGF对TBI患者并发ATC的预测价值通过绘制受试者工作特征(ROC)曲线进行分析。结果:中、重度组患者血清C3AR1、AngⅡ、VEGF水平高于轻度组,且重度组高于中度组(均P<0.05)。TBI患者血清C3AR1、AngⅡ、VEGF水平与GCS评分呈负相关(均P<0.05)。与非ATC组比较,ATC组患者GCS评分降低(P<0.05)。ATC组患者血清C3AR1、AngⅡ、VEGF水平高于非ATC组(均P<0.05)。GCS评分、血清C3AR1、AngⅡ、VEGF是TBI患者并发ATC的独立影响因素(均P<0.05)。血清C3AR1、AngⅡ、VEGF联合检测的曲线下面积(AUC)高于各自单独检测的AUC(均P<0.05)。结论:TBI患者血清C3AR1、AngⅡ、VEGF水平升高,与病情严重程度和ATC发生有关,三者联合检测对TBI患者并发ATC具有一定预测价值。
Abstract:
Objective:To investigate the changes and detection significance of serum complement 3a receptor 1 (C3AR1),angiotensin Ⅱ (AngⅡ) and vascular endothelial growth factor (VEGF) levels in patients with traumatic brain injury (TBI).Methods:A total of 88 TBI patients were selected and divided into mild group (28 cases),moderate group (33 cases) and severe group (27 cases) according to the Glasgow Coma Scale (GCS).The TBI patients were also divided into acute traumatic coagulopathy (ATC) group (41 cases) and non-ATC group (47 cases) based on whether ATC occurred.The serum levels of C3AR1,AngⅡ and VEGF were detected by ELISA.The correlation between serum levels of C3AR1,AngⅡ,VEGF and GCS scores in TBI patients was analyzed by Spearman method.Logistic regression analysis was used to analyze the influencing factors of ATC in TBI patients.The predictive value of serum C3AR1,AngⅡ and VEGF for ATC in TBI patients was analyzed by ROC curve.Results:The serum levels of C3AR1,AngⅡ and VEGF in moderate and severe groups were higher than those in mild group,and the levels in severe group were higher than those in moderate group (all P<0.05).The serum levels of C3AR1,AngⅡ and VEGF in TBI patients were negatively correlated with GCS scores (all P<0.05).Compared with non-ATC group,the GCS scores of ATC group were lower (P<0.05).The serum levels of C3AR1,AngⅡ and VEGF in ATC group were higher than those in non-ATC group (all P<0.05).GCS scores,serum C3AR1,AngⅡ and VEGF were independent influencing factors for ATC in TBI patients (all P<0.05).The AUC of combined detection of serum C3AR1,AngⅡand VEGF was higher than that of individual detection (all P<0.05).Conclusion:The serum levels of C3AR1,AngⅡ and VEGF in TBI patients are elevated,which are related to the severity of the disease and the occurrence of ATC.Combined detection of the three has certain predictive value for ATC in TBI patients.
