摘要:
自主神经功能障碍是自身免疫性脑炎(AE)的常见表现之一,可导致呼吸、心血管、消化、泌尿多个系统紊乱,对AE患者生活质量有较大影响。AE相关自主神经功能障碍的临床表现、发病机制较为复杂,自身免疫在其中起到关键作用,主流学说包括断连机制学说、兴奋/抑制比模型学说、轴索损伤学说、神经内分泌学说。该病目前缺乏公认的诊断标准,评估工具和测评指标包括汗液分泌功能检查、直立倾斜试验、Ewing试验、心率变异性。AE相关自主神经功能障碍以免疫治疗为基础,并辅以β-受体阻滞剂、吗啡、迷走神经电刺激、中医治疗等。对AE相关自主神经功能障碍的发病机制及诊治新进展进行综述。
Abstract:
Autonomic dysfunction is one of the usual symptom of autoimmune encephalitis(AE),which can lead to breath,cardiovascular,digestion and urology system disorders,greatly impact on the quality of life of patients with AE.Clinical manifestations and pathogenesis of AE-related autonomic dysfunction are complex,where autoimmune plays a key role.The mainstream theories include disconnection mechanism theory,excitatory inhibitory ratio model theory,axonal injury theory,and neuroendocrine theory.At present,there is no recognized diagnostic criteria for this disease.Its evaluation tools and evaluation indicators include sweat secretion function testing,head-up tilt testing,Ewing test,and heart rate variability.The treatment for AE-related autonomic dysfunction is based on immunotherapy,assisted by β-blockers,morphine,vagus nerve electrical stimulation,traditional Chinese medicine treatment,etc.Herein,new progress in pathogenesis,diagnosis and treatment of AE-related to autonomic dysfunction were reviewed.
