摘要:
目的:探讨视网膜中央静脉阻塞(CRVO)继发黄斑水肿患者超敏C-反应蛋白(hs-CRP)、细胞间黏附分子-1(ICAM-1)表达水平及检测意义。方法:选取165例CRVO患者为研究组,根据是否继发黄斑水肿将患者分为CRVO继发黄斑水肿组(100例)和单纯CRVO组(65例)。另选取同期眼部无异常的体检健康者50例为对照组。比较对照组和研究组血清hs-CRP、ICAM-1水平。根据黄斑水肿程度将CRVO继发黄斑水肿患者分为轻度水肿组(32例)、中度水肿组(41例)和重度水肿组(27例)。比较不同黄斑水肿程度CRVO继发黄斑水肿患者血清hs-CRP、ICAM-1水平。采用Spearman法分析血清hs-CRP、ICAM-1水平与CRVO继发黄斑水肿患者黄斑水肿程度的相关性。CRVO患者继发黄斑水肿的影响因素采用Logistic回归分析。血清hs-CRP、ICAM-1对CRVO患者继发黄斑水肿的预测价值通过绘制受试者工作特征(ROC)曲线进行评估。结果:与对照组比较,研究组血清hs-CRP、ICAM-1水平升高(均P<0.05)。CRVO继发黄斑水肿患者血清hs-CRP、ICAM-1水平随黄斑水肿程度增加依次升高(均P<0.05)。血清hs-CRP、ICAM-1水平与患者黄斑水肿程度呈正相关(均P<0.05)。CRVO继发黄斑水肿组患者中有糖尿病史、视网膜内层结构存在紊乱的比例以及血清hs-CRP、ICAM-1水平高于单纯CRVO组(均P<0.05)。糖尿病史、视网膜内层结构紊乱以及血清hs-CRP、ICAM-1是CRVO患者继发黄斑水肿的影响因素(均P<0.05)。血清hs-CRP、ICAM-1两者联合预测CRVO患者继发黄斑水肿的曲线下面积(AUC)大于两者单独检测的AUC(均P<0.05)。结论:CRVO继发黄斑水肿患者血清hs-CRP及ICAM-1水平升高,与黄斑水肿程度有关,两者联合对CRVO患者继发黄斑水肿具有较高的预测价值。
Abstract:
Objective:To investigate the expression levels of high-sensitivity C-reactive protein(hs-CRP)and intercellular adhesion molecule-1(ICAM-1)in patients with macular edema secondary to central retinal vein occlusion(CRVO)and to explore the significance of their detection.Methods:A total of 165 patients with CRVO were selected as the study group and divided into two subgroups:the CRVO with macular edema group(100 cases)and the simple CRVO group(65 cases).Additionally,50 healthy individuals with no ocular abnormalities were selected as the control group.Serum levels of hs-CRP and ICAM-1 were compared between the control and study groups.Patients with macular edema secondary to CRVO were further divided into three groups based on the severity of macular edema:mild edema group(32 cases),moderate edema group(41 cases)and severe edema group(27 cases).Serum levels of hs-CRP and ICAM-1 were compared among patients with different severities of macular edema.Spearman correlation analysis was used to assess the correlation between serum hs-CRP and ICAM-1 levels and the severity of macular edema in patients with macular edema secondary to CRVO.Logistic regression analysis was performed to identify the influencing factors of macular edema in CRVO patients.Receiver operating characteristic(ROC)curves were plotted to evaluate the predictive value of serum hs-CRP and ICAM-1 for macular edema in CRVO patients.Results:Compared with the control group,serum levels of hs-CRP and ICAM-1 were elevated in the study group(all P<0.05).In patients with macular edema secondary to CRVO,serum hs-CRP and ICAM-1 levels increased progressively with the severity of macular edema(all P<0.05).Serum hs-CRP and ICAM-1 levels were positively correlated with the severity of macular edema(all P<0.05).Patients in the CRVO with macular edema group had a higher proportion of diabetes history,retinal inner layer structural disorder,and elevated serum hs-CRP and ICAM-1 levels compared to those in the simple CRVO group(all P<0.05).Diabetes history,retinal inner layer structural disorder,and serum hs-CRP and ICAM-1 were identified as influencing factors for macular edema in CRVO patients(all P<0.05).The AUC for the combined prediction of macular edema using serum hs-CRP and ICAM-1 was greater than that of individual predictors(all P<0.05).Conclusion:Elevated serum levels of hs-CRP and ICAM-1 are associated with the severity of macular edema in patients with CRVO.Combined detection of these two biomarkers provides a higher predictive value for macular edema in CRVO patients.
