摘要:
目的:探讨在脊髓损伤并发尿路感染(UTI)患者血清干扰素-γ诱导蛋白10(IP-10)、诱骗受体3(DcR3)水平变化及检测意义。方法:选取脊髓损伤患者196例,根据是否发生UTI将患者分为UTI组(91例)和非UTI组(105例)。ELISA法测定血清IP-10、DcR3水平。采用Pearson法分析UTI组患者血清IP-10、DcR3水平的相关性; 采用Logistic回归分析脊髓损伤患者发生UTI的影响因素; 血清IP-10、DcR3对脊髓损伤患者发生UTI的预测价值通过绘制受试者工作特征(ROC)曲线进行分析。结果:UTI组患者保留导尿、肺部感染、外伤损伤比例及WBC、IL-1、IL-17、TNF-α水平高于非UTI组,RBC水平低于非UTI组(均P<0.05)。UTI组患者血清IP-10、DcR3水平高于非UTI组(均P<0.05)。UTI组患者血清IP-10、DcR3水平呈正相关(P<0.05)。血清IP-10、DcR3是脊髓损伤患者发生UTI的独立影响因素(均P<0.05)。血清IP-10、DcR3及两者联合预测脊髓损伤患者发生UTI的曲线下面积为(AUC)分别为0.804、0.813、0.887,两者联合预测的AUC高于血清IP-10、DcR3单独预测的AUC(均P<0.05)。结论:脊髓损伤并发UTI患者血清IP-10、DcR3水平升高,两者联合对脊髓损伤患者发生UTI具有一定预测效能。
Abstract:
Objective:To investigate the changes in serum interferon-γ inducible protein 10(IP-10)and decoy receptor 3(DcR3)levels in patients with spinal cord injury complicated by urinary tract infection(UTI),and to explore the significance of their detection.Methods:A total of 196 patients with spinal cord injury were selected and divided into two groups:the UTI group(91 cases)and the non-UTI group(105 cases).Serum IP-10 and DcR3 levels were measured using ELISA.Pearson correlation analysis was used to assess the correlation between serum IP-10 and DcR3 levels in the UTI group.Logistic regression analysis was performed to identify the influencing factors of UTI in spinal cord injury patients.Receiver operating characteristic(ROC)curves were plotted to analyze the predictive value of serum IP-10 and DcR3 for UTI in spinal cord injury patients.Results:The UTI group had higher proportions of indwelling catheterization,pulmonary infection and traumatic injury,as well as higher levels of white blood cells(WBC),interleukin-1(IL-1),IL-17,and tumor necrosis factor-α(TNF-α)compared to the non-UTI group.In contrast,the red blood cell(RBC)level was lower in the UTI group than in the non-UTI group(all P<0.05).Serum IP-10 and DcR3 levels were significantly higher in the UTI group than in the non-UTI group(all P<0.05).A positive correlation was observed between serum IP-10 and DcR3 levels in the UTI group(P<0.05).Serum IP-10 and DcR3 were identified as independent influencing factors for UTI in spinal cord injury patients(all P<0.05).The AUC for predicting UTI in spinal cord injury patients using serum IP-10,DcR3,and the combination of both were 0.804,0.813 and 0.887,respectively.The combined prediction of IP-10 and DcR3 had a higher AUC than individual predictions(all P<0.05).Conclusion:Elevated serum IP-10 and DcR3 levels are observed in spinal cord injury patients with UTI,and the combination of these two markers provides a certain predictive efficacy for UTI in spinal cord injury patients.
