目的:探讨半导体激光联合浓替硝唑含漱液治疗牙龈炎的临床价值。方法:选取154例牙龈炎患者,随机分为对照组(浓替硝唑含漱液治疗)和研究组(半导体激光联合浓替硝唑含漱液治疗),各77例。比较两组患者治疗效果,治疗前后龈沟液白细胞介素-1β(IL-1β)、前列腺素E2(PGE2)、C-反应蛋白(CRP)、可溶性细胞间黏附分子(sICAM-1)水平以及牙周参数,治疗后牙龈红肿及疼痛程度。记录治疗期间不良反应。结果:研究组有效率高于对照组(P<0.05)。治疗后,两组龈沟液IL-1β、PGE2CRP、sICAM-1水平及龈沟出血指数(SBI)、牙龈指数(GI)、牙菌斑指数(PLI)、牙周袋深度(PD)低于治疗前,且研究组龈沟液IL-1β、PGE2、CRP、sICAM-1水平及SBI、GI、PLI、PD低于对照组(均P<0.05)。研究组治疗后牙龈红肿及疼痛程度分级低于对照组(均P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:半导体激光联合浓替硝唑含漱液能够缓解牙龈炎患者炎症反应,减轻红肿及疼痛程度,改善牙周参数,治疗效果较好。
Abstract:
Objective:To explore the clinical value of semiconductor laser combined with concentrated metronidazole mouthwash in the treatment of gingivitis.Methods:One hundred and fifty-four patients with gingivitis were randomly assigned to either a control group(treated with concentrated metronidazole mouthwash)or a study group(treated with semiconductor laser combined with concentrated metronidazole mouthwash),with 77 patients in each group.The therapeutic effects,levels of gingival crevicular fluid interleukin-1β(IL-1β),prostaglandin E2(PGE2),C-reactive protein(CRP)and soluble intercellular adhesion molecule-1(sICAM-1),as well as periodontal parameters,were compared between the two groups before and after treatment.The degree of gingival redness and swelling,pain level after treatment,and adverse reactions during treatment were also recorded.Results:The study group exhibited a higher treatment efficacy rate compared to the control group(all P<0.05).After treatment,the levels of gingival crevicular fluid IL-1β,PGE2,CRP and sICAM-1,as well as the Gingival Bleeding Index(SBI),Gingival Index(GI),Plaque Index(PLI)and probing depth(PD)were lower than those before treatment in both groups,and the study group had significantly lower levels of these parameters compared to the control group(all P<0.05).The degree of gingival redness and swelling and pain level after treatment were lower in the study group than in the control group(all P<0.05).No significant difference was observed in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of semiconductor laser and concentrated metronidazole mouthwash can effectively alleviate inflammatory responses in patients with gingivitis,reduce redness and swelling,and pain levels,and improve periodontal parameters,with better therapeutic outcomes.
