摘要:
目的:探讨微小RNA-132(miR-132)、微小RNA-432(miR-432)与肺结核病情程度的关系及诊断价值分析。方法:选取94例肺结核患者作为研究对象。根据患者病情程度将其分为重症组(50例)和轻症组(44例),实时聚合酶链式反应检测两组血清中miR-132和miR-432的表达水平。观察两组患者的一般资料,并分析患者血清中miR-132、miR-432表达水平与其病情程度的相关性和诊断价值分析。结果:重症组患者血清miR-132低于轻症组、miR-432高于轻症组(均P<0.05)。Kendall分析发现miR-432与肺结核病情程度呈正相关关系(OR=0.539,P<0.05)。miR-132与肺结核病情程度呈负相关关系(OR=-0.714,P<0.05),差异具有统计学意义。Logistic多因素回归分析显示,血清miR-132、miR-432是影响肺结核患者病情程度的独立危险因素(均P<0.05)。ROC结果显示,血清miR-132、miR-432对诊断肺结核患者病情程度的曲线下面积(AUC)分别为0.793、0.808,对应灵敏度分别为64.00%、66.00%,特异性分别为95.00%、97.70%。二者联合诊断的AUC为0.824,灵敏度和特异度分别为70.00%和100.00%。结论:血清miR-132和miR-432是肺结核患者的独立危险因素,并且miR-432与肺结核患者的病情程度呈正相关,miR-132与病情程度呈负相关,因此在诊断肺结核患者病情程度的过程中,动态检测相关影响因素的变化可以为临床医生诊断病情提供依据,具有较高的诊断价值。
Abstract:
Objective:To investigate the relationship between microRNA-132(miRNA-132)and microRNA-432(miRNA-432)and the degree of tuberculosis and to analyse the diagnostic value.Methods:Ninety-four cases of pulmonary tuberculosis patients were selected as study subjects.The patients were divided into the severe group(50 cases)and the mild group(44 cases)according to the degree of their disease,and the expression levels of miR-132 and miR-432 in the serum of the two groups were detected by real-time polymerase chain reaction.The general data of the patients in the two groups were observed,and the correlation between the expression levels of miR-132 and miR-432 in the serum of the patients and the degree of their illnesses were analysed,as well as the diagnostic value.Results:Serum miR-132 was lower than that of the mild group and miR-432 was higher than that of the mild group in the severe group(all P<0.05).Kendall's analysis showed that miR-432 was positively correlated with the degree of TB disease(OR=0.539,P<0.05).miR-132 was negatively correlated with the degree of TB disease(OR=-0.714,P<0.05),and the difference was statistically significant and the difference was statistically significant.logistic multifactorial regression analysis showed that serum miR-132 and miR-432 were independent risk factors affecting the degree of condition of TB patients(all P<0.05).ROC results showed that the area under the curve(AUC)of serum miR-132 and miR-432 on the diagnosis of the degree of condition of TB patients were respectively 0.793 and 0.808,corresponding to a sensitivity of 64.00% and 66.00%,and a specificity of 95.00% and 97.70%,respectively.The AUC for the combined diagnosis of the two was 0.824,with a sensitivity and specificity of 70.00% and 100.00%,respectively.Conclusion:Serum miR-132 and miR-432 are independent risk factors for TB patients,and miR-432 is positively correlated with the degree of disease in TB patients,while miR-132 is negatively correlated with the degree of disease.Therefore,in the process of diagnosing the degree of disease in patients with TB,the dynamic detection of changes in the relevant influencing factors can provide clinicians with a basis for diagnosis of the disease,which has a high diagnostic value.Therefore,the dynamic detection of changes in relevant influencing factors can provide a basis for clinicians to diagnose the disease and has high diagnostic value.
