摘要:
目的:探讨半胱氨酸天冬氨酸蛋白酶3(Caspase-3)、微小RNA-431(miR-431)、可溶性血管细胞黏附分子-1(sVCAM-1)在突发性耳聋患者血清中的水平变化及其与耳蜗功能的相关性。方法:选取收治的突发性耳聋患者100例为疾病组,另选同期体检健康者50例为对照组。根据是否存在耳蜗性听力下降或损失分别判定疾病组患者为耳蜗功能异常(35例)或耳蜗功能正常(65例)。比较两组以及疾病组不同耳蜗功能患者血清Caspase-3、miR-431、sVCAM-1水平。采用Spearman法分析血清Caspase-3、miR-431、sVCAM-1水平与患者耳蜗功能的相关性。采用Logistic回归分析突发性耳聋患者耳蜗功能异常的影响因素。结果:疾病组血清Caspase-3、miR-431及sVCAM-1水平高于对照组(均P<0.05)。耳蜗功能异常患者血清Caspase-3、miR-431、sVCAM-1水平高于耳蜗功能正常患者(均P<0.05)。血清Caspase-3、miR-431、sVCAM-1水平与患者耳蜗功能呈负相关(均P<0.05)。耳蜗功能异常患者有上呼吸道感染史、有心血管合并症、重度和极重度听力损失的占比高于耳蜗功能正常患者(均P<0.05)。血清Caspase-3、miR-431、sVCAM-1是突发性耳聋患者耳蜗功能异常的独立影响因素(均P<0.05)。结论:突发性耳聋患者血清Caspase-3、miR-431及sVCAM-1水平升高,与耳蜗功能有关,是突发性耳聋患者耳蜗功能异常的影响因素。
Abstract:
Objective:To explore the changes in serum levels of Caspase-3,miR-431 and sVCAM-1 in patients with sudden deafness and their correlation with cochlear function.Methods:A total of 100 patients diagnosed with sudden sensorineural hearing loss were selected as the disease group,and 50 healthy individuals from the same period were selected as the control group.Patients in the disease group were classified as having cochlear dysfunction(35 cases)if they exhibited cochlear hearing loss or impairment,and as having normal cochlear function(65 cases)if they did not.The levels of Caspase-3,miR-431 and sVCAM-1 in the serum of the two groups and patients with different cochlear functions within the disease group were compared.Spearman method was used to analyze the correlation between the levels of Caspase-3,miR-431 and sVCAM-1 in the serum and the cochlear function of the patients.Logistic regression analysis was used to identify the influencing factors of cochlear dysfunction in patients with sudden sensorineural hearing loss.Results:The levels of Caspase-3,miR-431 and sVCAM-1 in the serum of the disease group were higher than those in the control group(all P<0.05).The levels of Caspase-3,miR-431 and sVCAM-1 in the serum of patients with cochlear dysfunction were higher than those with normal cochlear function(all P<0.05).The levels of Caspase-3,miR-431 and sVCAM-1 in the serum were negatively correlated with the cochlear function of the patients(all P<0.05).The proportion of patients with a history of upper respiratory tract infection,cardiovascular comorbidities,and severe to profound hearing loss were higher in patients with cochlear dysfunction than in those with normal cochlear function(all P<0.05).Serum Caspase-3,miR-431 and sVCAM-1 were independent influencing factors for cochlear dysfunction in patients with sudden sensorineural hearing loss(all P<0.05).Conclusion:The levels of Caspase-3,miR-431 and sVCAM-1 in the serum of patients with sudden sensorineural hearing loss are elevated and are related to cochlear function,making them influencing factors for cochlear dysfunction in patients with sudden sensorineural hearing loss.
