摘要:
目的:探讨血清叉头框D1(FOXD1)在鼻咽癌(NPC)患者中的表达及其与放疗后复发转移的关系。方法:选取本院NPC患者235例为NPC组,另选取同期本院体检健康志愿者246例为对照组。比较两组一般资料及血清FOXD1水平。分析血清FOXD1表达与NPC患者临床病理特征的关系。对化疗后NPC患者随访3年,记录复发转移情况。采用Cox回归分析NPC患者放疗后复发转移的影响因素。采用Spearman法分析血清FOXD1水平与肿瘤分化程度及EB病毒感染的相关性。绘制受试者工作特征(ROC)曲线分析血清FOXD1对NPC患者放疗后复发转移的预测价值。结果:NPC组FOXD1水平高于对照组(P<0.05)。NPC患者血清FOXD1表达与肿瘤分化程度、TNM分期、颈部淋巴结转移、肿瘤浸润深度、颅底侵犯、EB病毒感染有关(均P<0.05)。肿瘤低分化、EB病毒感染阳性及FOXD1高表达是NPC患者放疗后复发转移的独立危险因素(均P<0.05)。血清FOXD1水平与肿瘤分化程度呈负相关,与EB病毒感染呈正相关(均P<0.05)。FOXD1对NPC患者放疗后复发转移具有一定的预测价值(P<0.05)。结论:FOXD1在NPC患者血清中表达水平升高,与临床病理特征有关,是患者放疗后复发转移的危险因素,对放疗后复发转移具有一定的预测价值。
Abstract:
Objective:To explore the expression of serum forkhead box D1(FOXD1)in nasopharyngeal carcinoma(NPC)patients and its relationship with recurrence and metastasis after radiotherapy.Methods:A total of 235 NPC patients were selected as the NPC group,and another 246 healthy volunteers from the same period were selected as the control group.General data and serum FOXD1 levels of the two groups were compared.The relationship between serum FOXD1 expression and the clinical pathological characteristics of NPC patients was analyzed.The NPC patients were followed up for 3 years after chemotherapy,and the recurrence and metastasis were recorded.Cox regression analysis was used to analyze the influencing factors of recurrence and metastasis after radiotherapy in NPC patients.The Spearman method was used to analyze the correlation between serum FOXD1 levels and tumor differentiation and EB virus infection.The ROC curve was plotted to analyze the predictive value of serum FOXD1 for recurrence and metastasis after radiotherapy in NPC patients.Results:The FOXD1 level in the NPC group was higher than that in the control group(P<0.05).The expression of serum FOXD1 in NPC patients was correlated with tumor differentiation,TNM staging,cervical lymph node metastasis,tumor infiltration depth,skull base invasion,and EB virus infection(all P<0.05).Poor tumor differentiation,positive EB virus infection,and high expression of FOXD1 were independent risk factors for recurrence and metastasis after radiotherapy in NPC patients(all P<0.05).Serum FOXD1 levels were negatively correlated with tumor differentiation and positively correlated with EB virus infection(all P<0.05).FOXD1 had a certain predictive value for recurrence and metastasis after radiotherapy in NPC patients(P<0.05).Conclusion:The expression level of FOXD1 in the serum of NPC patients is increased and is related to clinical pathological characteristics.It is a risk factor for recurrence and metastasis after radiotherapy and has a certain predictive value for post-radiotherapy recurrence and metastasis.
