摘要:
目的:探讨可溶性血管细胞黏附分子-1(sVCAM-1)、中性粒细胞与淋巴细胞比值(NLR)在过敏性紫癜(HSP)患儿血清中的变化及检测意义。方法:选取接受诊治的HSP患儿86例(HSP组),另选取同时期体检健康儿童101例(对照组)。检测中性粒细胞、淋巴细胞及血清sVCAM-1,计算NLR。比较对照组和HSP组NLR、sVCAM-1水平变化。比较HSP组不同疾病分期患儿NLR、sVCAM-1水平变化。随访6个月,根据肾脏病变或复发情况将患儿分为预后不良组(14例)和预后良好组(72例)。比较HSP组不同预后患儿NLR与sVCAM-1水平变化,分析NLR与sVCAM-1水平对HSP患儿预后不良的预测价值。采用Pearson法分析HSP组NLR与sVCAM-1水平的相关性。结果:与对照组比较,HSP患儿NLR、sVCAM-1水平更高(均P<0.05)。在HSP组中,急性期患儿NLR、sVCAM-1水平高于恢复期患儿(均P<0.05)。预后不良组NLR、sVCAM-1水平高于预后良好组(均P<0.05)。NLR联合sVCAM-1预测患儿预后不良的AUC为0.712,高于NLR与sVCAM-1单独预测的AUC(均P<0.05)。NLR与sVCAM-1水平呈正相关(P<0.05)。结论:HSP患儿sVCAM-1和NLR水平升高,与病情活动性和预后有关,两者联合对患儿预后预测具有一定价值。
Abstract:
Objective:To investigate the changes of soluble vascular cell adhesion molecule-1(sVCAM-1)and neutrophil-to-lymphocyte ratio(NLR)in the serum of children with Henoch-Schönlein purpura(HSP)and their significance.Methods:A total of 86 children with HSP(HSP group)and 101 healthy children(control group)were selected.Neutrophils,lymphocytes and serum sVCAM-1 were detected and NLR was calculated.The levels of NLR and sVCAM-1 in control group and HSP group were compared.The levels of NLR and sVCAM-1 in children with different disease stages in HSP group were compared.The patients were followed up for 6 months and divided into poor prognosis group(14 cases)and good prognosis group(72 cases)according to kidney lesions or recurrence.The levels of NLR and sVCAM-1 in children with different prognosis in HSP group were compared,and the value of NLR and sVCAM-1 levels in predicting poor prognosis of children with HSP was analyzed.The correlation between NLR and sVCAM-1 levels in HSP group was analyzed by Pearson method.Results:Compared with the control group,the levels of NLR and sVCAM-1 were higher in the HSP group(all P<0.05).In the HSP group,the levels of NLR and sVCAM-1 in the acute stage were higher than those in the convalescent stage(all P<0.05).The levels of NLR and sVCAM-1 in the poor prognosis group were higher than those in the good prognosis group(all P<0.05).The AUC of NLR combined with sVCAM-1 in predicting poor prognosis was 0.712,which was higher than that of NLR and sVCAM-1 alone(all P<0.05).NLR was positively correlated with sVCAM-1 level(P<0.05).Conclusion:The levels of sVCAM-1 and NLR are increased in children with HSP,which are related to the disease activity and prognosis,and the combination of the two has certain value in predicting the prognosis of children.
