摘要:
目的: 探讨房颤(AF)患者射频消融术(RFA)后血浆微小RNA-21(miR-21)、微小RNA-150(miR-150)水平变化及其检测价值。方法:选取86例AF患者为观察对象,按RFA术后AF是否复发分为复发组(n=22)、非复发组(n=64)。采用实时荧光定量聚合酶链式反应(PCR)法测定血浆miR-21、miR-150水平,对比受试者手术前后以及复发组、未复发组血浆miR-21、miR-150表达差异,采用Spearman相关性分析RFA术后血浆miR-21、miR-150水平与AF复发的相关性; 采用受试者工作特征(ROC)曲线分析RFA术后血浆miR-21、miR-150水平对AF复发的预测价值。结果:RFA术后血浆miR-21水平低于术前(P<0.05),miR-150水平高于术前(P<0.05)。复发组RFA术后血浆miR-21水平高于未复发组(P<0.05),miR-150水平低于未复发组(P<0.05)。Spearman相关分析发现,RFA术后血浆miR-21水平与复发情况呈正相关(P<0.05),miR-150水平与复发情况呈负相关(P<0.05)。ROC曲线分析显示,RFA术后血浆miR-21、miR-150水平及联合检测对AF复发均有预测效能(均P<0.05),曲线下面积(AUC)分别为0.783、0.675、0.812,其中联合检验预测效能最高,敏感度、特异性分别为86.36%、67.19%。结论:AF患者RFA术后血浆miR-21水平降低,miR-150水平升高,且其术后miR-21、miR-150水平与AF复发存在相关性,可通过联合检测患者RFA术后血浆miR-21、miR-150水平预测AF复律后复发情况。
Abstract:
Objective:To explore the changes and detection value of plasma microRNA-21(miR-21)and microRNA-150(miR-150)levels in patients with atrial fibrillation(AF)after radiofrequency ablation(RFA).Methods:86 patients with AF were selected as the observation subjects.According to whether AF recurred after RFA,they were divided into recurrence group(n=22)and non-recurrence group(n=64).The levels of plasma miR-21 and miR-150 were measured by real-time fluorescence quantitative polymerase chain reaction(PCR).The differences in expressions of plasma miR-21 and miR-150 were compared among the subjects before and after surgery and between recurrence group and non-recurrence group.Spearman correlation was used to analyze the correlation between plasma miR-21 and miR-150 levels after RFA and AF recurrence.Receiver operating characteristic(ROC)curve was applied to analyze the predictive value of plasma miR-21 and miR-150 levels on AF recurrence after RFA.Results:The level of plasma miR-21 after RFA was lower than that before surgery,while the level of miR-150 was higher than that before surgery(all P<0.05).The plasma miR-21 level in recurrence group after RFA was higher,while the miR-150 level was lower compared to non-recurrence group(all P<0.05).Spearman correlation analysis found that plasma miR-21 level after RFA was positively correlated with recurrence,and miR-150 level was negatively correlated with recurrence(all P<0.05).ROC curve analysis showed that plasma miR-21,miR-150 levels and combined detection after RFA had predictive efficiency on AF recurrence(all P<0.05),and the areas under curves(AUC)were 0.783,0.675 and 0.812 respectively.The combined test had the highest predictive efficiency,with sensitivity of 86.36% and specificity of 67.19%.Conclusion:The level of plasma miR-21 is decreased and the level of miR-150 is increased in AF patients after RFA,and the levels of miR-21 and miR-150 after surgery are correlated with AF recurrence.Combined detection of plasma miR-21 and miR-150 levels after RFA can predict the recurrence of AF after cardioversion.
