摘要:
目的:探讨毛细支气管炎患儿糖皮质激素治疗前后血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、miR-302e、核因子-κB(NF-κB)mRNA水平变化及意义。方法:选取毛细支气管炎患儿108例为研究对象,均行糖皮质激素治疗。比较糖皮质激素治疗前后患儿血清IL-6、TNF-α、miR-302e、NF-κB mRNA水平变化。采用Pearson法分析IL-6、TNF-α、miR-302e、NF-κB mRNA间的相关性。根据糖皮质激素治疗效果将患儿分为有效组和无效组,比较不同疗效患儿一般资料及生化指标。分析IL-6、TNF-α、miR-302e、NF-κB mRNA水平对糖皮质激素治疗效果的影响,以及它们对毛细支气管炎患儿糖皮质激素治疗无效的预测价值。结果:糖皮质激素治疗后患儿血清IL-6、TNF-α、NF-κB mRNA水平低于治疗前,miR-302e水平高于治疗前(均P<0.05)。Pearson相关性分析显示,患儿血清IL-6与TNF-α、NF-κB mRNA水平呈正相关(r=0.512、0.564,均P<0.05),与miR-302e呈负相关(r=-0.573,P<0.05); 患儿血清TNF-α与NF-κB mRNA水平呈正相关(r=0.521,P<0.05),与miR-302e水平呈负相关(r=-0.559,P<0.05); 患儿血清miR-302e与NF-κB mRNA水平呈负相关(r=-0.571,P<0.05)。无效组患儿血清IL-6、TNF-α、NF-κB mRNA水平,以及重度病情患儿占比高于有效组,miR-302e水平低于有效组(均P<0.05)。Logistic回归分析结果显示,血清IL-6、TNF-α、miR-302e、NF-κB mRNA水平异常以及病情严重程度(重度)是毛细支气管炎患儿糖皮质激素治疗无效的危险因素(均P<0.05)。血清IL-6、TNF-α、miR-302e、NF-κB mRNA联合检测对毛细支气管炎患儿糖皮质激素治疗无效的预测价值高于上述指标单独预测。结论:经糖皮质激素治疗后,毛细支气管炎患儿血清IL-6、TNF-α、NF-κB mRNA水平降低,miR-302e水平升高,四者水平异常是毛细支气管炎患儿糖皮质激素治疗无效的危险因素,联合检测对毛细支气管炎患儿糖皮质激素治疗无效的预测价值更高。
Abstract:
Objective:To investigate the changes of serum interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),miR-302e,nuclear factor-κB(NF-κB)mRNA levels and their significance in children with bronchiolitis before and after glucocorticoid treatment.Methods:108 children with bronchiolitis were selected as the study subjects,all of whom were treated with glucocorticoid.The levels of IL-6,TNF-α,miR-302e and NF-κB mRNA were compared before and after treatment with glucocorticoid.The correlation between IL-6,TNF-α,miR-302e and NF-κB mRNA was analyzed by Pearson method.According to the effect of glucocorticoid treatment,the children were divided into effective group and ineffective group,and the general information and biochemical indexes of the children with different curative effects were compared.The effects of IL-6,TNF-α,miR-302e and NF-κB mRNA levels on clinical efficacy of glucocorticoid therapy in children with bronchiolitis,and the predictive value of failure of glucocorticoid therapy in children with bronchiolitis were investigated.Results:The serum levels of IL-6,TNF-α and NF-κB mRNA in children after glucocorticoid treatment were lower than those before treatment,and the level of miR-302e was higher than that before treatment(all P<0.05).Pearson correlation analysis showed that serum IL-6 was positively correlated with TNF-α and NF-κB mRNA levels(r=0.512,0.564,both P<0.05),and negatively correlated with miR-302e(r=-0.573,P<0.05); serum TNF-α was positively correlated with NF-κB mRNA level(r=0.521,P<0.05),and negatively correlated with miR-302e level(r=-0.559,P<0.05); serum miR-302e was negatively correlated with NF-κB mRNA level(r=-0.571,P<0.05).The levels of serum IL-6,TNF-α,NF-κB mRNA and the proportion of children with severe disease in the ineffective group were higher than those in the effective group,and the level of miR-302e was lower than that in the effective group(all P<0.05).Logistic regression analysis showed that abnormal serum IL-6,TNF-α,miR-302e,NF-κB mRNA levels and severity of the disease were risk factors for failure of glucocorticoid therapy in children with bronchiolitis(all P<0.05).The combined detection of serum IL-6,TNF-α,miR-302e and NF-κB mRNA in children with bronchiolitis has higher predictive value than the above indexes alone.Conclusion:After treatment with glucocorticoids,serum levels of IL-6,TNF-α and NF-κB mRNA decreased and miR-302e level increased in children with bronchiolitis.Abnormal levels of the four are risk factors for the failure of glucocorticoid therapy in children with bronchiolitis.Combined detection has a higher value in predicting the failure of glucocorticoid therapy in children with bronchiolitis.
