摘要:
目的:探讨多不饱和脂肪酸ω-3(ω-3 PUFA)、ω-6(ω-6 PUFA)和ω-3/ω-6与冠状动脉疾病的相关性。方法:采用回顾性队列研究,选取心内科收治的322例疑似冠心病患者作为研究对象,根据冠脉造影结果分为冠心病组(166例)与非冠心病组(156例),采集静脉血测定ω-3 PUFA、ω-6 PUFA水平,计算ω-3/ω-6值。对比两组的ω-3 PUFA、ω-6 PUFA及ω-3/ω-6差异,并分析ω-3 PUFA、ω-6 PUFA、ω-3/ω-6与冠心病及冠脉病变程度的相关性。结果:与非冠心病组相比,冠心病组的男性占比更高,年龄更大,吸烟率、高血压及糖尿病占比更高,空腹血糖(FPG)、同型半胱氨酸(Hcy)、肌钙蛋白T(CTNT)及肌酸激酶同工酶(CK-MB)水平更高(P<0.05)。冠心病组的ω-3 PUFA和ω-3/ω-6均显著低于非冠心病组,ω-6 PUFA水平显著高于非冠心病组(P<0.05)。Logistic回归分析显示ω-3 PUFA、ω-6 PUFA、ω-3/ω-6对冠心病均影响显著,并且ω-6 PUFA为危险因素,ω-3 PUFA和ω-3/ω-6为保护因素(P<0.05)冠心病患者的Gensini积分与ω-6 PUFA呈正相关,与ω-3 PUFA、ω-3/ω-6呈负相关(均P<0.05),其中与ω-3/ω-6的相关性最强(r=-0.231,P<0.05)。结论:ω-3 PUFA、ω-6 PUFA、与冠脉病变密切相关,临床测定ω-3 PUFA、ω-6 PUFA及ω-3/ω-6值可为门诊及体检人群的冠心病筛查提供参考依据。
Abstract:
Objective:To discussion on the correlation between polyunsaturated fatty acids ω-3(ω-3 PUFA),ω-6(ω-6 PUFA)and ω-3/ω-6 with coronary artery disease.Methods:A retrospective cohort study was used to select 322 patients with suspected coronary heart disease admitted to the cardiology department as the study object.According to the results of coronary angiography,they were divided into coronary heart disease group(166 cases)and non coronary heart disease group(156 cases),and venous blood was collected for determination ω-3 PUFA,ω-6 PUFA and ω-3/ω-6 calculation values.The ω-3 PUFA,ω-6 PUFA and ω-3/ω-6 difference were compared between the two groups,and the ω-3 PUFA,ω-6 PUFA and ω-3/ω-6 correlation with coronary heart disease and degree of coronary artery disease were analyzed.Results:Compared with the non coronary heart disease group,the coronary heart disease group had a higher proportion of male,older age,higher smoking rate,hypertension and diabetes,and higher levels of fasting blood glucose(FPG),homocysteine(Hcy),troponin T(CTNT)and creatine kinase-MB(CK-MB)(all P<0.05).The ω-3 PUFA and ω-3/ω-6 of coronary heart disease group were significantly lower than those in non coronary heart disease group,and the level of ω-6 PUFA was significantly higher than that of non coronary heart disease group(all P<0.05).Logistic regression analysis shows that ω-3 PUFA,ω-6 PUFA and ω-3/ω-6 has significant effect on coronary heart disease,and ω-6 PUFA was a risk factor,ω-3 PUFA and ω-3/ω-6 were protective factors(all P<0.05).The Gensini score and ω-6 PUFA was positively correlated,but with ω-3 PUFA and ω-3/ω-6 were negatively correlated(all P<0.05),among which ω-3/ω-6 had the strongest correlation(r=-0.231,P<0.05).Conclusion:ω-3 PUFA and ω-6 PUFA are closely related to coronary artery disease,which is measured clinically ω-3 PUFA,ω-6 PUFA and ω-3/ω-6 values can provide reference for coronary heart disease screening in outpatient and physical examination population.
