摘要:
目的:探讨甲氧明对老年冠心病患者前列腺电切术中血流动力学及认知功能的影响。方法:98例合并冠心病的老年前列腺电切术患者,采用随机数字表法将其分为对照组(给予常规麻醉)和研究组(在对照组基础上给予甲氧明治疗)各49例,统计两组不同时间的血流动力学指标,记录两组麻醉时间、手术时间、首次排气时间、拔管时间、出室时间、术后住院时间及不良反应,观察两组术前1 d、术后1、3、7 d认知功能。结果:T1时,两组SpO2、MAP、心率、CO、SVR水平比较,差异无统计学意义(均P>0.05); T2、T3、T4、T5时,研究组SpO2、MAP、心率、CO、SVR水平高于对照组,差异有统计学意义(均P<0.05)。研究组麻醉时间、首次排气时间、出室时间均低于对照组,差异有统计学意义(均P<0.05); 两组手术时间、术后住院时间比较,差异无统计学意义(均P>0.05)。术前1 d、术后7 d,两组认知功能评分比较,差异无统计学意义(均P>0.05); 术后1 d及术后3 d,研究组认知功能评分高于对照组,差异有统计学意义(均P<0.05)。研究组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论:甲氧明能够有效稳定老年冠心病患者前列腺电切术中的血流动力学,加速认知功能恢复、且可降低不良反应,值得临床推广应用。
Abstract:
Objective:To investigate the effect of methoxamine on hemodynamics in elderly patients with coronary heart disease during electroprostatectomy.Methods:A total of 98 cases patients underwent electroprostatectomy were selected and divided into control group(given routine anesthesia)and study group(given methoxamine treatment on the basis of the control group)with 49 cases each by random number table method.The hemodynamics of the two groups at different time were analyzed.Anesthesia time,operation time,first exhaust time,extubation time,exit time,postoperative hospitalization time and adverse reactions of the two groups were recorded.Cognitive function of the two groups was observed 1 day before surgery and 1,3,7 days after surgery.Results:There were no significant differences in SpO2,MAP,heart rate,CO and SVR in T1 between the two groups(all P>0.05).In T2,T3,T4 and T5,SpO2,MAP,heart rate,CO,SVR levels in the study group were higher than those in the control group(all P<0.05).Anesthesia time,first exhaust time and exit time in the study group were lower than those in the control group(all P<0.05).There was no significant difference in operation time,extubation time and postoperative hospital stay between the two groups(all P>0.05).There was no significant difference in cognitive function score between the two groups 1 and 7 days before surgery(all P>0.05).The cognitive function score of the study group was higher than that of the control group 1 and 3 days after surgery(all P<0.05).The adverse reactions in the study group were lower than those in the control group(P<0.05).Conclusion:Methoxamine can effectively stabilize hemodynamics during electroprostatectomy in elderly patients with coronary heart disease,and reduce adverse reactions,which can be used in clinical application.
