目的:观察脑小血管病(CSVD)患者血清甲壳质酶蛋白40(YKL-40)、视锥蛋白样蛋白1(VILIP-1)表达特征,探讨两者与认知障碍的相关性。方法:收集CSVD患者共63例作为观察组,收集体检证实为无明显器质性疾病的成人(健康成人)血清标本63例作为对照组,应用ELISA法检测血清中YKL-40和VILIP-1的表达。应用蒙特利尔认知评估量表(MoCA)评估认知障碍情况。结果:观察组血清中YKL-40和VILIP-1的表达量明显高于对照组(均P<0.05),观察组MoCA评分明显低于对照组(P<0.05)。观察组中YKL-40和VILIP-1在有无高血压、有无糖尿病病史、有无认知障碍分组中的表达差异有统计学意义(均P<0.05),观察组中MoCA在有无高血压、有无糖尿病病史分组评分中的差异有统计学意义(均P<0.05)。观察组血清中YKL-40和VILIP-1的表达具有正相关性(r=0.62,P=0.032),YKL-40和MoCA 评分(r=-0.61,P=0.025)、VILIP-1和MoCA评分(r=-0.68,P=0.011)均具有负相关性。结论:CSVD患者血清中存在YKL-40和VILIP-1高表达的状态,不仅与病变的形成有关,还与认知障碍相关。
Abstract:
Objective:To observe the expression characteristics of serum chitin enzymes protein 40(YKL-40)and visinin-like protein 1(VILIP-1)in patients with cerebral small vessel disease(CSVD),and to explore their correlation with cognitive impairment.Methods:A total of 63 CSVD patients were collected as the observation group,and 63 healthy adults were collected as the control group.The expressions of YKL-40 and VILPI-1 were detected by ELISA.Cognitive impairment was evaluated by MoCA.Results:Expression levels of YKL-40 and VILIP-1 in the observation group were significantly higher than those in the control group(all P<0.05).MOCA score in the observation group was significantly lower than that in the control group(P<0.05).In the observation group,there were significant differences in the expressions of YKL-40 and VILIP-1 in the groups with or without hypertension,diabetes history and cognitive impairment(all P<0.05),and there were significant differences in MoCA scores in the groups with or without hypertension and diabetes history(all P<0.05).Positive correlation was found between YKL-40 and VILIP-1 in the observation group(r=0.62,P=0.032).Negative correlation was found between YKL-40 and MOCA score(r=-0.61,P=0.025),VILIP-1 and MOCA score(r=-0.68,P=0.011).Conclusion:High expression of serum YKL-40 and VILIP-1 is related not only to the formation of lesions,but also to cognitive impairmen in CSVD patients.
