摘要:
目的:观察儿童呼吸困难评分(PRAM)在支气管哮喘患儿急性发作时对病情的评估作用,并分析PRAM与患儿肺通气功能的相关性,为临床诊疗提供理论依据。方法:研究对象为支气管哮喘急性发作患儿,包括哮喘急性轻度(轻度组)、中度(中度组)及重度患儿(重度组)各60例。比较各组患者PRAM及肺通气功能指标水平,并分析PRAM与患儿各项肺通气功能指标的相关性。结果:哮喘急性发作轻度患儿PRAM评分为(2.17±0.83)分,哮喘急性发作中度患儿PRAM评分为(4.82±1.00)分,哮喘急性发作重度患儿PRAM评分为(8.17±0.81)分,且各组间比较差异均具有统计学意义(均P<0.05)。与哮喘急性发作轻度患儿相比,哮喘急性发作中度及重度患儿第一秒用力呼气末容积占比(FEV1%)、用力肺活量占比(FVC%)、最大呼气流量占比(PEF%)、用力呼气50%肺活量时的瞬间流量占比(FEF50%)水平均显著降低(均P<0.05),且各组间比较差异均具有统计学意义(均P<0.05); 相关性分析结果显示,哮喘急性发作轻度、中度、重度患儿PRAM评分均与肺通气功能相关指标(FEV1%、FVC%、PEF%、FEF50%)呈负相关(均P<0.05)。结论:在患儿支气管哮喘急性发作时,PRAM评分对患儿病情严重程度具有较好的评估价值,与临床诊断分级标准一致性较高,同时PRAM评分与患儿肺通气功能检测结果趋同性较高,相关性分析显示二者之间存在显著负相关,应用PRAM评分对支气管哮喘患儿急性发作时的病情进行判断,具有很好的临床诊疗价值,有助于早期进行及时有效干预,提高患儿生活质量。
Abstract:
Objective:To analyze the role of Preschool Respiratory Assessment Measure(PRAM)in the condition of children with bronchial asthma during acute attack,and to analyze the correlation between PRAM and pulmonary ventilation function of children,so as to provide theoretical basis for clinical diagnosis and treatment.Methods:The children with an acute exacerbation of bronchial asthma were enrolled,including 60 children with mild acute asthma(mild group),60 with moderate acute asthma(moderate group),and 60 with severe acute asthma(severe group).The levels of PRAM and pulmonary ventilation function indexes in each group were compared,and the correlation between PRAM and pulmonary ventilation function indexes was analyzed.Results:The PRAM score was(2.17±0.83),(4.82±1.00),and(8.17±0.81)in children with mild,moderate,and severe acute asthma exacerbation respectively,and there were statistically significant differences between the groups(all P<0.05).Compared with the children with mild acute asthma,the levels of FEV1%,FVC%,PEF% and FEF50% in the children with moderate and severe acute asthma were significantly lower(all P<0.05).Correlation analysis showed that PRAM scores were negatively correlated with pulmonary ventilation function(FEV1%,FVC%,PEF%,FEF50%)in children with mild,moderate and severe acute asthma(all P<0.05).Conclusion:In children with acute exacerbation of bronchial asthma,PRAM score has a good evaluation value for the severity of the disease,which is highly consistent with the clinical diagnostic grading standard.At the same time,PRAM score has a high convergence with the detection results of the pulmonary ventilation function of children.Correlation analysis shows that there is a significant negative correlation between them.The application of PRAM score in the judgment of the condition of children with bronchial asthma at acute attack has a good clinical diagnosis and treatment value,which is helpful for early timely and effective intervention and improving the quality of life of children.
