摘要:
目的:分析免疫性血小板减少症(ITP)患儿T淋巴细胞亚群、甲状腺自身抗体、微小RNA-106b-5p(miR-106b-5p)和微小RNA-181a-5p(miR-181a-5p)表达水平及临床意义。方法:以158例ITP患儿为ITP组,依据临床分型不同分为慢性ITP组(34例)、持续性ITP组(60例)和新诊断ITP组(64例)。另选取同期健康儿童102例为对照组。所有患儿均给予静脉滴注人免疫球蛋白及口服醋酸泼尼松片等常规治疗,均治疗5 d。比较ITP组与对照组全血T淋巴细胞亚群(CD3+、CD4+、CD8+)细胞及血清甲状腺自身抗体[甲状腺球蛋白抗体(TGAb)、甲状腺过氧化物酶抗体(TPOAb)]、miR-106b-5p、miR-181a-5p水平。比较慢性ITP组、持续性ITP组和新诊断ITP组患儿全血T淋巴细胞亚群及血清TGAb、TPOAb、miR-106b-5p、miR-181a-5p水平。比较ITP患儿治疗前、治疗5 d后全血T淋巴细胞亚群及血清TGAb、TPOAb、miR-106b-5p、miR-181a-5p水平。结果:与对照组比较,ITP组全血CD3+、CD4+细胞及血清miR-181a-5p水平更低,全血CD8+细胞及血清TGAb、TPOAb、miR-106b-5p水平更高(均P<0.05)。慢性ITP组全血CD3+、CD4+细胞及血清miR-181a-5p水平低于持续性ITP组和新诊断ITP组,全血CD8+细胞及血清TGAb、TPOAb、miR-106b-5p水平高于持续性ITP组和新诊断ITP组(均P<0.05)。与治疗前比较,治疗5 d后ITP患儿全血CD3+、CD4+细胞及血清miR-181a-5p水平升高,全血CD8+细胞及血清TGAb、TPOAb、miR-106b-5p水平降低(均P<0.05)。结论:ITP患儿存在T淋巴细胞亚群失衡、甲状腺自身抗体和miR-106b-5p高表达以及miR-181a-5p低表达情况。治疗期间积极检测患儿全血T淋巴细胞亚群及血清甲状腺自身抗体、miR-106b-5p、miR-181a-5p水平,有助于患儿的疗效评估。
Abstract:
Objective:To analyze the expression levels and clinical significance of T lymphocyte subsets,thyroid autoantibodies,microRNA-106b-5p(miR-106b-5p)and microRNA-181a-5p(miR-181a-5p)in children with immune thrombocytopenia(ITP).Methods:A total of 158 children with ITP were enrolled as the ITP group.According to the clinical classification,they were divided into chronic ITP group(34 cases),persistent ITP group(60 cases),and newly diagnosed ITP group(64 cases).A total of 102 healthy children were selected as the control group.All children with ITP were given conventional treatment such as intravenous infusion of human immunoglobulin and oral prednisone acetate tablets for 5 days.The levels of whole blood T lymphocyte subsets(CD3+,CD4+,CD8+)cells,serum thyroid autoantibodies [thyroglobulin antibody(TGAb),thyroid peroxidase antibody(TPOAb)],miR-106b-5p and miR-181a-5p were compared between ITP group and control group.The whole blood T lymphocyte subsets and serum TGAb,TPOAb,miR-106b-5p and miR-181a-5p levels were compared among chronic ITP,persistent ITP and newly diagnosed ITP groups.The T lymphocyte subsets in whole blood and the levels of serum TGAb,TPOAb,miR-106b-5p and miR-181a-5p were compared before and 5 days after treatment.Results:Compared with the control group,the ITP group had significantly lower levels of CD3+ and CD4+ cells in whole blood and serum miR-181a-5p,and significantly higher levels of CD8+ cells,TGAb,TPOAb,and miR-106b-5p(all P<0.05).The levels of CD3+ and CD4+ cells in whole blood and serum miR-181a-5p in chronic ITP group were lower than those in persistent and newly diagnosed ITP groups,while the levels of CD8+ cells in whole blood and serum TGAb,TPOAb and miR-106b-5p were higher than those in persistent and newly diagnosed ITP groups(all P<0.05).Compared with those before treatment,the levels of CD3+ and CD4+ cells in whole blood and serum miR-181a-5p were increased,and the levels of CD8+ cells in whole blood and serum TGAb,TPOAb and miR-106b-5p were decreased in ITP children after 5 days of treatment(all P<0.05).Conclusion:The imbalance of T lymphocyte subsets,high expression of thyroid autoantibodies and miR-106b-5p,and low expression of miR-181a-5p may exist in children with ITP.Active detection of T lymphocyte subsets in whole blood,serum thyroid autoantibodies,miR-106b-5p and miR-181a-5p levels during treatment is helpful to evaluate the therapeutic effect.
