摘要:
目的:探究血清缺氧诱导因子-1α(HIF-1α)、前列腺素E2(PGE2)与动脉瘤性蛛网膜下腔出血的关系及其对脑血管痉挛的预测价值。方法:选择90例动脉瘤性蛛网膜下腔出血患者为研究组,另选同期100例体检健康者为对照组,检测两组血清HIF-1α、PGE2水平,分析HIF-1α、PGE2水平与研究组患者病理特征的关系,经数字减影血管造影或经颅多普勒血流分析发现脑血管痉挛患者30例,无脑血管痉挛患者60例,对比脑血管痉挛组和无脑血管痉挛组患者血清HIF-1α、PGE2水平,采用受试者工作特征(ROC)曲线分析术后3 d患者血清HIF-1α、PGE2、HIF-1α+PGE2水平对脑血管痉挛的诊断价值。结果:①研究组患者血清HIF-1α、PGE2水平均明显高于对照组(均P<0.05); ②研究组血清HIF-1α、PGE2水平与患者的Hunt-Hess分级、Fisher分级有关(均P<0.05); ③脑血管痉挛组患者的血清HIF-1α、PGE2水平明显高于无脑血管痉挛组患者(均P<0.05); ④通过绘制血清HIF-1α、PGE2水平对脑血管痉挛预测ROC曲线,计算其AUC分别为0.749、0.811,联合检测的AUC为0.854。结论:动脉瘤性蛛网膜下腔出血患者血清HIF-1α、PGE2水平均升高,且能够有效预测脑血管痉挛的发生。
Abstract:
Objective:To explore the relationship between serum hypoxia-inducible factor-1α(HIF-1α),prostaglandin E2(PGE2)and aneurysmal subarachnoid hemorrhage,and its predictive value for cerebral vasospasm.Methods:A total of 90 patients with aneurysmal subarachnoid hemorrhage were selected as the study group,and 100 healthy individuals at the same time were selected as the control group.Serum levels of HIF-1α and PGE2 in the two groups were detected,and the relationship between the levels of HIF-1α,PGE2 and the pathological characteristics of the patients in the study group was analyzed.The patients were diagnosed with cerebral vasospasm by digital subtraction angiography or transcranial Doppler flow analysis,and there were 60 cases without cerebral vasospasm and 30 cases with cerebral vasospasm.The serum levels of HIF-1α and PGE2 in patients with cerebral vasospasm and those without cerebral vasospasm were compared.ROC curve was used to analyze the diagnostic value of serum HIF-1α,PGE2,and HIF-1α+PGE2 levels in patients 3 days after surgery.Results:①The levels of serum HIF-1α and PGE2 in the study group were significantly higher than those in the control group(all P<0.05).②The serum levels of HIF-1α and PGE2 in the study group were related to the Hunt-Hess classification and Fisher classification of the patients(all P<0.05).③The levels of serum HIF-1α and PGE2 in patients with cerebral vasospasm were significantly higher than those in patients without cerebral vasospasm(all P<0.05).④By plotting the ROC curve of serum HIF-1α and PGE2 levels to predict cerebral vasospasm,the AUCs were calculated to be 0.749 and 0.811 respectively,and the diagnostic AUC of combined detection was 0.854.Conclusion:Serum HIF-1α and PGE2 levels are elevated in patients with aneurysmal subarachnoid hemorrhage,which can effectively predict the occurrence of cerebral vasospasm.
