摘要:
目的:探析DeBakeyⅠ型主动脉夹层和其他类型主动脉夹层低血钾的差异及临床意义。方法:回顾性选取64例Ⅰ型夹层和165例其他类型夹层的患者病例资料,对血钾值和其他各类临床数据进行统计学分析。根据入院时主动脉经 CT 血管造影(CTA)结果显示分为Ⅰ型、Ⅱ型、Ⅲ型(DeBakey分型)。结果:Ⅰ型组和其他类型夹层组间冠心病、总胆固醇、低密度脂蛋白、C反应蛋白(CRP)比较差异有统计学意义(均P<0.05)。Logistic回归分析发现,低血钾(OR:3.195,95%CI:1.257～8.119)及冠心病(OR:6.72,95%CI:3.225～14.005)与Ⅰ型夹层显著相关; 且在发病24 h内就诊的Ⅰ型患者低血钾发生率(50%)明显高于其他类型组(32.1%)(P<0.05); 在低血钾分组中Ⅰ型主动脉夹层的血钾值(3.00±0.28)低于其他类型主动脉夹层的血钾值(3.17±0.23,P<0.05)。 结论:Ⅰ型主动脉夹层较其他类型夹层低血钾严重程度更深,发生低血钾的风险更高,考虑发病早期的低血钾有助于提示Ⅰ型主动脉夹层及严重并发症的发生。
