摘要:
目的:探讨下肢静脉瓣膜功能不全相关风险因素。方法:选择慢性下肢静脉疾病患者1073例,根据2022年欧洲血管外科学会下肢慢性静脉疾病管理临床实践指南将静脉疾病临床-病因-解剖-病理-生理(CEAP)分级中C3至C6级定义为慢性静脉瓣膜功能不全(CVI),将C0至C2级定义为慢性静脉瓣膜功能不全早期(CVI早期)。比较CVI早期与CVI患者一般资料。采用Logistic回归分析下肢静脉瓣膜功能不全的独立危险因素。采用受试者工作特征(ROC)曲线分析各独立危险因素对CVI的诊断价值。结果:CVI早期与CVI患者性别、年龄、身高、体重、体重指数、病程、久站时间、久坐时间、吸烟史、腿部外伤史、下肢静脉血栓史、糖尿病史、风湿病史、患者静脉疾病临床症状评分(HASTI)及静脉临床严重程度(VCSS)评分比较差异有统计学意义(均P<0.05)。Logistic回归分析结果显示,性别、年龄、体重、病程、久站时间、下肢静脉血栓史及糖尿病史是下肢静脉瓣膜功能不全的独立危险因素。ROC曲线分析显示,年龄>50岁、体重>68 kg、病程>12年及久站时间>6 h对CVI具有一定的诊断价值。结论:慢性下肢静脉瓣膜功能不全的独立危险因素包括男性、年龄>50岁、体重>68 kg、病程>12年、每日站立时间>6 h、下肢静脉血栓病史及糖尿病史。针对性地对相关危险因素进行早期预防可降低或延缓疾病的进展。
Abstract:
Objective:To investigate the risk factors associated with lower extremity venous valve insufficiency.Methods:A total of 1073 patients with chronic lower extremity venous disease were selected.According to the 2022 European Society of Vascular Surgery clinical practice guidelines for the management of chronic venous disease of the lower extremities,C3 to C6 were defined as chronic venous valve insufficiency(CVI),and C0 to C2 were defined as early stages of CVI(early CVI)in CEAP classification.The general data of patients with early CVI and CVI were compared.Logistic regression was used to analyze independent risk factors for lower extremity venous valve insufficiency.ROC curve was used to analyze the diagnostic value of each independent risk factor for CVI.Results:There were significant differences in gender,age,height,weight,body mass index,course of disease,long standing time,sedentary time,smoking,leg trauma,lower limb venous thrombosis,diabetes,rheumatism,HASTI score and VCSS score between CVI patients and early CVI patients(all P<0.05).Logistic regression analysis showed that sex,age,weight,course of disease,long standing time,history of lower limb venous thrombosis and diabetes were independent risk factors for lower extremity venous valve insufficiency.ROC curve analysis showed that age >50 years,weight >68 kg,course of disease >12 years and long standing time >6 hours had certain diagnostic value for CVI.Conclusion:The independent risk factors for chronic lower extremity venous valve insufficiency include male sex,age >50 years,weight >68 kg,course of disease >12 years,daily standing time >6 hours,history of venous thrombosis of lower limbs and history of diabetes.Targeted early prevention of associated risk factors can reduce or delay disease progression.
