摘要:
目的:目的:探究血清胆红素、尿酸(UA)联合同型半胱氨酸(Hcy)检测对冠心病的诊断价值。方法:选取冠心病患者94例作为冠心病组,同时选择同期体检健康的志愿者43例作为对照组。比较两组总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)、UA、Hcy水平,比较不同冠状动脉病变支数患者及不同Gensini评分患者TBIL、DBIL、IBIL、UA、Hcy水平。采用受试者工作特征(ROC)曲线分析胆红素、UA、Hcy对冠心病的诊断价值。结果:冠心病组TBIL、DBIL、IBIL水平低于对照组,UA、Hcy水平高于对照组(均P<0.05)。随着冠状动脉病变支数和Gensini评分的增加,TBIL、DBIL、IBIL水平依次下降,UA、Hcy水平依次上升(均P<0.05)。ROC曲线分析表明,TBIL、DBIL、IBIL、UA、Hcy单独及联合检测均对冠心病有一定的诊断价值,且联合检测的敏感度和准确性高于单独检测(均P<0.05)。结论:胆红素水平下降,UA、Hcy水平上升,均与冠心病加重明显相关,且胆红素、UA联合Hcy检测对冠心病诊断具有较高价值。
Abstract:
Objective:To investigate the diagnostic value of serum bilirubin and uric acid(UA)combined with homocysteine(Hcy)in the detection of coronary heart disease.Methods:A total of 94 patients with coronary heart disease were selected as the coronary heart disease group,and 43 healthy volunteers who received physical examination during the same period were selected as the control group.Total bilirubin(TBIL),direct bilirubin(DBIL),indirect bilirubin(IBIL),UA,and Hcy were compared between two groups.TBIL,DBIL,IBIL,IBIL,UA,and Hcy levels in patients with different coronary lesions and in patients with different Gensini scores were compared.ROC curve was used to analyze the diagnostic value of bilirubin,UA,and Hcy for coronary artery disease.Results:TBIL,DBIL and IBIL levels were lower in coronary artery disease group than those in control group,and UA and Hcy levels were higher than those in control group(all P<0.05).As the number of coronary artery lesions and the Gensini score increased,TBIL,DBIL and IBIL levels decreased sequentially,and the levels of UA and Hcy increased sequentially(all P<0.05).ROC curve analysis showed that TBIL,DBIL,IBIL,UA,and Hcy in individual and combined detection had diagnostic value for coronary heart disease,and the sensitivity and accuracy of the combined detection were higher than those of the individual detection(all P<0.05).Conclusion:Decrease of bilirubin level and increase of UA and Hcy levels are significantly related to the aggravation of coronary heart disease,and the detection of bilirubin and UA combined with Hcy is of high value in the diagnosis of coronary heart disease.
