摘要:
目的:探讨颈前路椎间盘切除减压融合术(ACDF)后颈长肌、颈伸肌形态学变化与改良日本矫形协会(mJOA)评分及颈部功能障碍指数(NDI)的相关性。方法:选取接受ACDF治疗的患者64例为研究对象,分别于术前、术后12个月采用mJOA评分及NDI评价颈椎功能状态,并进行MRI检查,测量C<sup>2~C<sup>7各椎间盘水平颈长肌截面积(AxCSA)和长短径线比值(RLS),依据层厚计算颈长肌容积,同时测量相同层面颈伸肌截面积(CESA)并将其与相应椎体截面积(VBA)的比值作为颈伸肌容积。比较手术前后颈长肌、颈伸肌的形态学变化,并与mJOA评分及NDI进行相关性分析。结果:术后12个月时,手术节段AxCSA和颈长肌容积较术前显著减少,RLS较术前显著增加(均P<0.05)。术前和术后12个月时CESA、CESA/VBA比较,差异无统计学意义(均P>0.05)。与术前比较,术后12个月时mJOA评分明显增加,而NDI显著下降(均P<0.05)。颈长肌容积与NDI呈负相关(P<0.05),与mJOA评分无相关性(P>0.05); CESA/VBA与mJOA评分和NDI无相关性(均P>0.05)。结论:ACDF后CESA和颈伸肌容积无明显改变,而手术节段相应的颈长肌形态变化显著,且颈长肌容积与术后NDI存在负相关性。
Abstract:
Objective:To investigate the correlation between morphological changes of longus colli and extensor of neck after anterior cervical discectomy and fusion(ACDF)and modified Japanese Orthopaedic Association(mJOA)score and Neck Disability Index(NDI).Methods:A total of 64 patients who were treated with ACDF were selected as the research objects.mJOA score and NDI were used to evaluate the functional status of cervical spine before and 12 months after operation.The cross-sectional area(AxCSA)and the ratio of long to short diameter(RLS)of longus colli at the levels of C<sup>2 to C<sup>7 intervertebral discs were measured by MRI examination,while the cervical extensor cross-sectional area(CESA)at the same level was measured,and the ratio of CESA to the corresponding vertebra body area(VBA)was taken as the volume of extensor of neck.The morphological changes of longus colli and extensor of neck before and after operation were compared,and the correlation with mJOA score and NDI was analyzed.Results:At 12 months after operation,AxCSA and the volume of longus colli at the surgical segment was significantly reduced compared with those before surgery,and RLS was significantly increased(all P<0.05).There was no significant difference between CESA and CESA/VBA before and 12 months after operation(all P>0.05).The mJOA score increased significantly and NDI decreased significantly at 12 months after operation compared with those before surgery(all P<0.05).The volume of longus cervical muscle was negatively correlated with NDI(P<0.05),but not with mJOA score(P>0.05); CESA/VBA was not correlated with mJOA score and NDI(all P>0.05).Conclusion:There is no significant change in CESA and the volume of extensor of neck after ACDF,but there is a significant change in the morphology of the corresponding longus colli at the operative segment,and there is a negative correlation between the volume of longus colli and postoperative NDI.
