摘要:
目的:分析传染性单核细胞增多症(IM)患儿血清Toll样受体2(TLR2)、TLR9与外周血T淋巴细胞亚群变化及临床意义。方法:回顾性分析106例IM患儿临床资料。记录并比较IM患儿急性期与恢复期血清TLR2、TLR9及外周血T淋巴细胞亚群(CD3+、CD4+、CD8+、CD4+/CD8+)变化情况。根据治疗7 d时外周血EB病毒脱氧核糖核酸(EBV-DNA)转阴情况将IM患儿分为转阴组和未转阴组,比较两组一般资料,以及治疗7 d时血清TLR2、TLR9及外周血T淋巴细胞亚群表达水平。分析治疗7 d时EBV-DNA载量与血清TLR2、TLR9及外周血T淋巴细胞亚群表达水平的关系。结果: IM患儿急性期血清TLR2、TLR9表达水平高于恢复期,外周血CD3+、CD8+表达水平高于恢复期,外周血CD4+及CD4+/CD8+表达水平则低于恢复期(均P<0.05)。治疗7 d时,106例IM患儿EBV-DNA转阴82例(77.36%),纳入转阴组; 未转阴24例(22.64%),纳入未转阴组。两组患儿性别、入院前病程及治疗前白细胞计数比较差异无统计学意义(均P>0.05); 未转阴组年龄低于转阴组,治疗前EBV-DNA载量高于转阴组(均P<0.05)。治疗7 d时,未转阴组血清TLR2、TLR9表达水平高于转阴组,外周血CD3+、CD8+表达水平高于转阴组,外周血CD4+及CD4+/CD8+低于转阴组(均P<0.05)。经Pearson相关性分析,发现EBV-DNA载量与血清TLR2、TLR9及外周血CD3+、CD8+表达水平呈正相关(r=0.381、0.362、0.319、0.347,均P<0.05),与外周血CD4+及CD4+/CD8+呈负相关(r=-0.360、-0.395,均P<0.05)。结论:血清TLR2、TLR9及外周血CD3+、CD4+、CD8+、CD4+/CD8+表达水平随IM患儿病情转归而变化,积极监测上述指标可辅助临床判断病情。
Abstract:
Objective:To analyze the changes and clinical significance of serum Toll-like receptor 2(TLR2),TLR9 and peripheral blood T lymphocyte subsets in children with infectious mononucleosis(IM).Methods:The clinical data of 106 children with IM were retrospectively analyzed.The changes of serum TLR2,TLR9 and peripheral blood T lymphocyte subsets(CD3+、CD4+、CD8+、CD4+/CD8+)were recorded and compared between acute phase and convalescent phase.According to the negative conversion of peripheral blood EB virus deoxyribonucleic acid(EBV-DNA)of children with IM after 7 days of treatment,they were divided into negative conversion group and non-negative conversion group.The general data,the expression levels of serum TLR2,TLR9 and peripheral blood T lymphocyte subsets were compared between the two groups after 7 days of treatment.Pearson correlation analysis was used to evaluate the relationship between EBV-DNA load and expression levels of serum TLR2,TLR9 and peripheral blood T lymphocyte subsets after 7 days of treatment.Results:The expression levels of serum TLR2,TLR9 and peripheral blood CD3+ and CD8+ in acute phase were higher than those in convalescent phase,while the peripheral blood CD4+ and CD4+/CD8+ were lower than those in convalescent phase(all P<0.05).After 7 days of treatment,among 106 children with IM,82 cases(77.36%)with negative conversion were included in negative conversion group,and 24 cases(22.64%)without negative conversion were included in non-negative conversion group.There was no significant difference in gender,course of disease before admission and white blood cell count before treatment between two groups(all P>0.05).The age in non-negative conversion group was lower than that in negative conversion group,while the EBV-DNA load before treatment was higher than that in negative conversion group(all P<0.05).After 7 days of treatment,the expression levels of serum TLR2,TLR9 and peripheral blood CD3+ and CD8+ in non-negative conversion group were higher than those in negative conversion group,while the peripheral blood CD4+ and CD4+/CD8+ were lower than those in negative conversion group(all P<0.05).Pearson correlation analysis showed that EBV-DNA load was positively correlated with expression levels of serum TLR2,TLR9 and peripheral blood CD3+ and CD8+(r=0.381,0.362,0.319,0.347,all P<0.05),and was negatively correlated with peripheral blood CD4+ and CD4+/CD8+(r=-0.360,-0.395,all P<0.05).Conclusion:The expression levels of serum TLR2,TLR9 and peripheral blood CD3+,CD4+,CD8+ and CD4+/CD8+ change with the outcome of children with IM.Active monitoring of the above indexes can assist clinical judgment.
