摘要:
目的:探究女性生殖器结核(FGT)不孕患者的超声特征及其与卵巢储备功能的相关性。方法:纳入FGT不孕患者84例,根据卵巢储备功能评估结果,将31例卵巢储备功能下降患者纳入异常组,53例正常患者纳入正常组。比较两组的超声指标,包括卵泡数(AFC)、卵巢体积、舒张末期血流速度(EDV)、基质血流峰值流速(PSV)。使用ROC曲线分析超声指标对FGT不孕患者卵巢储备功能下降的评估价值。使用二元Logistic回归分析影响FGT不孕患者卵巢储备功能的危险因素。结果:正常组的AFC、EDV、卵巢体积、PSV均高于异常组,差异具有统计学意义(均P<0.05)。AFC对FGT不孕患者卵巢储备功能下降的评估价值最高。异常组患者的月经异常发生率高于正常组,差异具有统计学意义(P<0.05)。AFC、EDV、卵巢体积、PSV、月经异常均与FGT不孕患者卵巢储备功能异常明显相关(均P<0.05)。结论:FGT不孕合并卵巢储备功能下降患者AFC、EDV、卵巢体积、PSV明显降低。临床中对于发生AFC、EDV、卵巢体积、PSV以及月经异常的FGT不孕患者,应注意监测患者的卵巢储备功能,及时开展治疗,以阻碍卵巢储备功能下降,降低卵巢早衰的发生风险。
Abstract:
Objective:To investigate the ultrasonographic features of infertile patients with female genital tuberculosis(FGT)and their correlation with ovarian reserve.Methods:A total of 84 infertile patients with FGT were included.According to the evaluation results of ovarian reserve,31 patients with decreased ovarian reserve were included in the abnormal group,and 53 normal patients were included in the normal group.The ultrasound indexes of the two groups were compared,including follicle count(AFC),ovarian volume,end-diastolic blood flow velocity(EDV),and peak stromal flow velocity(PSV).The ROC curve was used to analyze the evaluation value of ultrasound indicators for the decline of ovarian reserve in infertile patients with FGT.Binary Logistic regression was used to analyze the risk factors affecting ovarian reserve in infertile patients with FGT.Results:The AFC,EDV,ovarian volume and PSV of the normal group were higher than those of the abnormal group(all P<0.05).AFC had the highest value in assessing decreased ovarian reserve in infertile patients with FGT.The incidence of abnormal menstruation in the abnormal group was higher than that in the normal group(P<0.05).AFC,EDV,ovarian volume,PSV and abnormal menstruation were significantly correlated with abnormal ovarian reserve in infertile patients with FGT(all P<0.05).Conclusion:AFC,EDV,ovarian volume and PSV in infertile patients with FGT combined with decreased ovarian reserve were significantly decreased.In clinical practice,for FGT infertile patients with abnormal AFC,EDV,ovarian volume,PSV and menstruation,attention should be paid to monitoring the patient's ovarian reserve function,and timely treatment should be carried out to prevent the decline of ovarian reserve function and reduce the risk of premature ovarian failure.
