摘要:
目的:探讨妊娠期高血压疾病(HDP)孕妇血清和胎盘组织中血管内皮生长因子(VEGF)、糖基化终末产物(AGEs)、转化生长因子-β1(TGF-β1)表达水平变化情况,并分析其与病情严重程度的相关性。方法:选取90例HDP孕妇为研究对象,根据病情严重程度分为HDP组、轻度子痫前期组(轻度PE组)、重度子痫前期组(重度PE组),各组30例; 同期内另选取30例健康妊娠孕妇设为对照组。采集各组产妇分娩前血清和分娩后胎盘组织,免疫吸附法和免疫组化法分别检测VEGF、AGEs、TGF-β1表达情况,并采用Spearman相关性分析其与病情严重程度关系。结果:HDP组、轻度PE组、重度PE组孕妇血清VEGF水平低于对照组,血清AGEs、TGF-β1水平则高于对照组(均P<0.05); 重度PE组VEGF水平低于HDP组和轻度PE组,重度PE组AGEs、TGF-β1水平高于HDP组和轻度PE组,轻度PE组TGF-β1水平高于HDP组(均P<0.05)。HDP组、轻度PE组、重度PE组孕妇胎盘组织中VEGF阳性表达率均低于对照组,而AGEs、TGF-β1阳性表达率均高于对照组(均P<0.05); 重度PE组AGEs阳性表达率高于HDP组和轻度PE组,重度PE组TGF-β1阳性表达率高于轻度PE组(均P<0.05)。血清VEGF与妊娠期高血压疾病严重程度呈负相关,AGEs、TGF-β1则与疾病严重程度呈正相关(均P<0.05)。结论:HDP孕妇血清及胎盘组织中VEGF水平显著低于正常妊娠孕妇,与病情严重程度呈负相关; HDP孕妇血清及胎盘组织中AGEs、TGF-β1水平则显著高于正常妊娠孕妇,与病情严重程度呈正相关。三者与妊娠高血压疾病密切相关,在临床评估疾病进展中有重要意义。
Abstract:
Objective:To investigate the expression levels of vascular endothelial growth factor(VEGF),advanced glycation end products(AGEs)and transforming growth factor-β1(TGF-β1)in serum and placental tissue of pregnant women with hypertensive disorders of pregnancy(HDP),and to analyze their correlation with the severity of the disease.Methods:A total of 90 pregnant women with HDP were selected and divided into HDP group,mild preeclampsia group(mild PE group)and severe preeclampsia group(severe PE group)according to the severity of the disease,with 30 cases in each group.Another 30 healthy pregnant women were selected as the control group during the same period.Pre-delivery serum and post-delivery placental tissue of each group were collected,and the expression levels of VEGF,AGEs and TGF-β1 were detected by immunoadsorption and immunohistochemistry,respectively.The relationship between them and the severity of the disease was analyzed by Spearman correlation.Results:Serum VEGF levels in HDP group,mild PE group and severe PE group were lower than those in control group,while serum AGEs and TGF-β1 levels were higher than those in control group(all P<0.05).VEGF levels in severe PE group were lower than those in HDP group and mild PE group,AGEs and TGF-β1 levels in severe PE group were higher than those in HDP group and mild PE group,and TGF-β1 level in mild PE group was higher than that in HDP group(all P<0.05).The positive expression rate of VEGF in placenta of HDP group,mild PE group and severe PE group were lower than those of control group,while the positive expression rates of AGEs and TGF-β1 were higher than those of control group(all P<0.05).The positive expression rate of AGEs in severe PE group was higher than that in HDP group and mild PE group,and the positive expression rate of TGF-β1 in severe PE group was higher than that in mild PE group(all P<0.05).Serum VEGF was negatively correlated with the severity of hypertensive disorder in pregnancy,while AGEs and TGF-β1 were positively correlated with the severity of hypertensive disorder in pregnancy(all P<0.05).Conclusion:The levels of VEGF in serum and placental tissue of HDP pregnant women were significantly lower than those of normal pregnant women,which were negatively correlated with the severity of the disease.The levels of AGEs and TGF-β1 in serum and placental tissue of HDP pregnant women were significantly higher than those of normal pregnant women,which were positively correlated severe disease.All three are closely related to gestational hypertension and are of great significance in clinical assessment of disease progression.
