摘要:
目的:观察反复上呼吸道感染患儿血清中纤维胶凝蛋白3(FCN3)的表达情况,并分析其与患儿体液免疫功能的相关性及对疾病的预测价值。方法:选择反复上呼吸道感染患儿126例为观察组,选择同时期体检结果健康的儿童120例为对照组。采用酶联免疫吸附实验测定两组受试者及观察组患儿治疗前后血清FCN3、体液免疫指标(IgA、IgG、IgM)的表达水平。采用Pearson分析法分析反复上呼吸道感染患儿血清FCN3与IgA、IgG、IgM的相关性。采用受试者工作特征(ROC)曲线分析血清FCN3表达对反复上呼吸道感染发病的预测价值。结果:观察组患儿血清FCN3、IgA、IgG、IgM表达水平低于对照组(均P<0.05)。患儿治疗后血清中FCN3表达水平及免疫指标水平较治疗前提高(均P<0.05)。Pearson分析结果显示,FCN3表达水平与IgA、IgG、IgM呈正相关(r=0.531、0.436、0.499,均P<0.05)。ROC曲线分析显示,血清中FCN3预测反复上呼吸道感染发生的曲线下面积为0.752(95%CI:0.688~0.815),截断值为15.723 μg/ml,敏感度为96.0%,特异度为51.7%。结论:反复上呼吸道感染患儿血清中FCN3呈低表达,且与IgA、IgG、IgM的表达水平呈正相关,是预测反复上呼吸道感染较为良好的指标。
Abstract:
Objective:To observe the expression of ficolin-3(FCN3)in serum of children with recurrent upper respiratory tract infection,and to analyze its correlation with humoral immune function and its predictive value for disease.Methods: A total of 126 children with recurrent upper respiratory tract infection and 120 healthy children in the same period were selected as observation group and control group,respectively.Enzyme-linked immunosorbent assay was used to measure the expression levels of FCN3 and humoral immune indexes(IgA,IgG,IgM)in serum of the two groups and the observation group before and after treatment.Pearson analysis was used to analyze the correlation between serum FCN3 and IgA,IgG and IgM in children with recurrent upper respiratory tract infection.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of serum FCN3 expression for recurrent upper respiratory tract infections.Results:The expression levels of FCN3,IgA,IgG and IgM in the observation group were lower than those in the control group(all P<0.05).After treatment,the expression level of FCN3 and the levels of immune indexes in serum were higher than those before treatment(all P<0.05).Pearson analysis showed that FCN3 expression level was positively correlated with IgA,IgG and IgM(r=0.531,0.436 and 0.499,all P<0.05).ROC curve analysis showed that the area under the curve of FCN3 in serum to predict the occurrence of recurrent upper respiratory tract infection was 0.752(95%CI:0.688 to 0.815),cut-off value was 15.723 μg/ml,sensitivity was 96.0%,and specificity was 51.7%.Conclusion:FCN3 expression in serum of children with recurrent upper respiratory tract infection is low and positively correlated with the expression levels of IgA,IgG and IgM,which is a good predictor of recurrent upper respiratory tract infection.
