摘要:
目的:探讨血清核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)炎症小体活化Gasdermin D(GSDMD)蛋白介导的细胞焦亡在痛风性关节炎中的作用及其机制。方法:分别收集40例急性痛风性关节炎患者(试验组)和40例健康体检者(对照组); 采用血常规检测仪和全自动生化分析仪检测两组血清中生化指标(肾功、血脂)及白细胞计数(WBC)、血沉(ESR)、C反应蛋白(CRP)等炎性指标的含量。采用免疫印迹法检测法(Western blot)分别检测两组外周血单个核细胞中NLRP3、GSDMD、白介素1β(IL-1β)蛋白的表达量。通过酶联免疫吸附法(ELISA)检测两组血浆中IL-1β和白介素18(IL-18)的表达量。结果:与对照组相比,试验组中WBC、ESR、CRP水平较高,差异具有统计学意义(均P<0.05); 试验组细胞焦亡相关蛋白NLRP3、GSDMD、IL-1β及IL-18表达显著增加(均P<0.05); 试验组血尿酸水平、NLRP3、GSDMD与炎性因子(IL-1β及IL-18)表达量呈线性正相关(均P<0.05)。结论:NLRP3可能通过激活的GSDMD蛋白介导的细胞焦亡引起IL-1β 等炎症因子的释放,参与急性痛风性关节炎的免疫炎性反应。
Abstract:
Objective:To investigate the role and mechanism of serum nucleotide-binding oligomerization domain-like receptor protein 3(NLRP3)inflammasome-activated Gasdermin D(GSDMA)protein-mediated pyroptosis in gouty arthritis.Methods: A total of 40 patients with acute gouty arthritis(experimental group)and 40 healthy subjects(control group)were collected.Blood routine detector and automatic biochemical analyzer were used to detect serum biochemical indexes(renal function,blood lipids),white blood cell count(WBC),erythrocyte sedimentation rate(ESR),C-reactive protein(CRP)and other inflammatory indexes of the two groups.Western blot was used to detect the protein expression of NLRP3,GSDMA and IL-1β in the peripheral blood mononuclear cells.The expression levels of IL-1β and IL-18 in plasma were detected by enzyme-linked immunosorbent assay(ELISA).Results:Compared with the control group,the levels of WBC,ESR and CRP in the experimental group were higher,and the expression of pyroptosis-related proteins NLRP3,GSDMA,IL-1β and IL-18 in the experimental group significantly increased(all P<0.05).The serum uric acid level,NLRP3,GSDMA and the expression of inflammatory factors(IL-1β and IL-18)in the experimental group were linearly positively correlated(all P<0.05).Conclusion:NLRP3 may induce the release of inflammatory factors such as IL-1β through activated GSDMD protein-mediated pyroptosis,and participate in the immune inflammatory response of acute gouty arthritis.
