摘要:
目的:探讨振幅整合脑电图(AEEG)对病毒性脑膜炎婴幼儿预后的评估价值。方法:选取病毒性脑膜炎婴幼儿64例为病毒性脑膜炎组,选取同期非脑膜炎婴幼儿52例为非脑膜炎组。所有患儿入院24 h内完成AEEG检查。采用婴儿神经系统国际测验量表(Infanib)评估患儿早期运动发育情况,将病毒性脑膜炎患儿分为运动发育正常组(25例)、运动发育过渡组(21例)和运动发育异常组(18例)。采用Gesell量表评估患儿神经发育情况,计算发育商,将病毒性脑膜炎患儿分为神经发育正常组(45例)和神经发育异常组(19例)。比较病毒性脑膜炎组和非脑膜炎组患儿AEEG评分、Infanib评分以及发育商。比较不同运动发育程度和不同神经发育程度病毒性脑膜炎患儿AEEG评分。采用受试者工作特征(ROC)曲线分析AEEG评分对病毒性脑膜炎婴幼儿预后的评估价值。结果:病毒性脑膜炎组患儿AEEG评分中的连续性、睡眠周期、下边界振幅、波普带宽度与下边界振幅变化评分低于非脑膜炎组(均P<0.05)。病毒性脑膜炎组患儿Infanib评分和发育商低于非脑膜炎组(均P<0.05)。与运动发育正常组和运动发育过渡组相比,运动发育异常组连续性、睡眠周期、波普带宽度与下边界振幅变化评分降低(均P<0.05)。神经发育异常组患儿AEEG评分中的连续性、睡眠周期、下边界振幅和波普带宽度与下边界振幅变化评分低于神经发育正常组(均P<0.05)。ROC曲线分析显示,AEEG评分对病毒性脑膜炎婴幼儿预后具有较高的评估价值,其中曲线下面积(AUC)为0.888(95%CI:0.811~0.965,P<0.001),截断值为9.020分,敏感度为95.3%,特异度为71.1%。结论:病毒性脑膜炎婴幼儿AEEG评分较非脑膜炎婴幼儿呈降低趋势,AEEG对病毒性脑膜炎婴幼儿预后具有良好的评估价值。
Abstract:
Objective:To explore the value of amplitude-integrated electroencephalogram(AEEG)in evaluating prognosis of infants with viral meningitis.Methods:A total of 64 infants with viral meningitis were enrolled as viral meningitis group,while other 52 infants without meningitis during the same period were enrolled as non-meningitis group.All underwent AEEG detection within 24 hours after admission.The early motor development was evaluated by Infanib,and the infants with viral meningitis were divided into normal motor development group(25 cases),transition motor development group(21 cases)and abnormal motor development group(18 cases).Gesell scale was used to evaluate the neurodevelopment of infants,and the development quotient was calculated,and the infants with viral meningitis were divided into normal neurodevelopmental group(45 cases)and abnormal neurodevelopmental group(19 cases).AEEG score,infanib score and development quotient were compared between viral meningitis group and non-meningitis group.The AEEG scores of infants with viral meningitis between different degrees of motor development and between different degrees of neurodevelopment were compared.Receiver operating characteristic(ROC)curve was used to analyze the value of AEEG score in evaluating the prognosis of infants with viral meningitis.Results:The scores of AEEG(continuity,sleep cycle,amplitude of lower boundary,changes in band width and amplitude of lower boundary)in viral meningitis group were lower than those in non-meningitis group,and infanib score and development quotient were lower than those in non-meningitis group(all P<0.05).Compared with normal motor development group and transition motor development group,AEEG scores(continuity,sleep cycle,changes in band width and amplitude of lower boundary)in abnormal motor development group were decreased(all P<0.05).The scores of AEEG(continuity,sleep cycle,amplitude of lower boundary,changes in band width and amplitude of lower boundary)in abnormal neurodevelopment group were lower than those in normal neurodevelopment group(all P<0.05).ROC curve analysis showed that AEEG score had high value in evaluating the prognosis of infants with viral meningitis,and the area under the curve(AUC)was 0.888(95%CI:0.811-0.965,P< 0.001),the cut-off value was 9.020,the sensitivity was 95.3%,and the specificity was 71.1%.Conclusion:AEEG score in infant with viral meningitis is lower than that without meningitis.AEEG has a good value in evaluating the prognosis of infants with viral meningitis.
