摘要:
目的:检测先天性心脏病患儿血清微小RNA(miR)-148b-3p表达水平,探究其临床意义。方法:选取先天性心脏病患儿150例为研究对象(试验组),根据患儿情况又分为单纯房间隔缺损组(48例)、单纯室间隔缺损组(37例)以及复合组(65例)。选取同期进行健康体检的儿童150例为对照组。采用实时荧光定量PCR(qRT-PCR)检测两组儿童血清miR-148b-3p表达水平。采用全自动生化分析仪检测两组儿童血清肌酸激酶同工酶(CK-MB)水平。采用酶联免疫吸附(ELISA)法检测两组儿童血清中肌钙蛋白I(cTnI)水平。采用超声诊断仪检测左心室射血分数(LVEF)、左心室舒张末期容积指数(LVEDVI)、左心室收缩末期室壁应力(LVESWS)水平。采用Pearson法分析先天性心脏病患儿血清miR-148b-3p表达水平与心功能指标的相关性。采用受试者工作特征(ROC)曲线分析miR-148b-3p水平对先天性心脏病患儿的早期预测价值。结果:与对照组相比,试验组患儿miR-148b-3p、cTnI、CK-MB、LVEDVI、LVESWS水平显著升高,LVEF显著降低(均P<0.05)。单纯房间隔缺损组和单纯室间隔缺损组患儿血清miR-148b-3p水平显著高于复合组(均P<0.05),但单纯房间隔缺损组患儿血清miR-148b-3p水平与单纯室间隔缺损组比较,差异无统计学意义(P>0.05)。Pearson相关性分析显示,先天性心脏病患儿血清miR-148b-3p水平与cTnI、CK-MB、LVEDVI、LVESWS呈正相关,与LVEF呈负相关(均P<0.05)。ROC曲线分析显示,miR-148b-3p水平预测先天性心脏病患儿的曲线下面积为0.873,截断值为1.28,敏感度为81.33%,特异度为85.67%。结论:先天性心脏病患儿血清miR-148b-3p表达水平较高,且与先天性心脏病患儿心功能指标具有相关性,可以作为预测先天性心脏病患儿及评估其病情的参考指标。
Abstract:
Objective:To detect the serum miR-148b-3p expression level in children with congenital heart disease and explore its clinical significance.Methods:150 children with congenital heart disease were selected as the research object(experimental group).According to the situation of children,they were divided into simple atrial septal defect group(48 cases),simple ventricular septal defect group(37 cases)and combination group(65 cases).150 children who underwent physical examination in the same period were selected as the control group.The expression level of serum miR-148b-3p in the two groups was detected by qRT-PCR.The level of serum CK-MB was detected by automatic biochemical analyzer.The level of serum cTnI in the two groups was detected by ELISA.LVEF,LVEDVI and LVESWS were measured by ultrasonic diagnostic instrument.Pearson method was used to analyze the correlation between serum miR-148b-3p expression level and cardiac function indexes in children with congenital heart disease.The ROC curve was used to analyze the early predictive value of miR-148b-3p level in children with congenital heart disease.Results:Compared with the control group,the levels of miR-148b-3p,cTnI,CK-MB,LVEDVI and LVESWS in the experimental group were significantly higher and LVEF was significantly lower(all P<0.05).The level of serum miR-148b-3p in simple atrial septal defect group and simple ventricular septal defect group was significantly higher than that in combination group(all P<0.05),but there was no significant difference between simple atrial septal defect group and simple ventricular septal defect group(P>0.05).Pearson correlation analysis showed that the level of serum miR-148b-3p in children with congenital heart disease was positively correlated with cTnI,CK-MB,LVEDVI and LVESWS,and negatively correlated with LVEF(all P<0.05).ROC curve analysis showed that the area under the curve predicted by miR-148b-3p level in children with congenital heart disease was 0.873,the cut-off value was 1.28,the sensitivity was 81.33%,and the specificity was 85.67%.Conclusion:The expression level of serum miR-148b-3p in children with congenital heart disease is high,and it is correlated with the indexes of cardiac function in children with congenital heart disease.It can be used as a reference index for predicting children with congenital heart disease and evaluating their condition.
