摘要:
目的:分析支气管哮喘合并肺炎支原体(MP)感染患儿血清白介素-6(IL-6)、25羟基维生素D[25(OH)D]、肿瘤坏死因子α(TNF-α)的水平变化,为临床诊断提供依据。方法:回顾性选取120例支气管哮喘患儿作为研究对象。根据是否合并MP感染将所有患儿分为感染组(65例)和非感染组(55例),同期选择50例健康体检儿童作为对照组。比较三组的IL-6、25(OH)D、TNF-α水平、第一秒用力呼气容积(FEV1)、呼气流量峰值(PEF)、用力肺活量(FVC),采用Logistic回归分析IL-6、25(OH)D、TNF-α与支气管哮喘合并MP感染的关系,并应用受试者工作特征曲线(ROC)评估IL-6、25(OH)D、TNF-α对疾病的预测价值。结果:感染组的IL-6、TNF-α水平明显高于非感染组和对照组,且非感染组显著高于对照组,而观察组的25(OH)D水平明显低于非感染组和对照组,且非感染组显著低于对照组,差异有统计学意义(均P<0.05); 感染组、非感染组和对照组的FVC、PEF、FEV1比较差异无统计学意义(均P>0.05); Logistic回归显示,IL-6、25(OH)D、TNF-α水平是支气管哮喘合并MP感染的独立危险因素(均P<0.05); ROC结果显示,IL-6、25(OH)D、TNF-α的ACU均高于0.7,对支气管哮喘合并MP感染具有较高的预测价值; 感染组的TNF-α与IL-6呈正相关(r=0.969,P<0.001),TNF-α与25(OH)D呈正相关(r=0.984,P<0.001),IL-6与25(OH)D呈正相关(r=0.920,P<0.001)。结论:IL-6、25(OH)D、TNF-α与支气管哮喘合并MP感染密切相关,可作为评估支气管哮喘合并MP感染程度的重要指标。
Abstract:
Objective:To analyze the levels of serum interleukin-6(IL-6),25 hydroxyvitamin D [25(OH)D] and tumor necrosis factor-α(TNF-α)in children with bronchial asthma complicated with Mycoplasma pneumoniae(MP)infection,and to provide the basis for clinical diagnosis.Methods:120 children with bronchial asthma were selected as the research objects.According to whether they were complicated with MP infection,all children were divided into infection group(65 cases)and non infection group(55 cases).50 healthy children were selected as control group.The levels of IL-6,25(OH)D,TNF-α,forced expiratory volume in the first second(FEV1),peak expiratory flow(PEF),forced vital capacity(FVC)in the three groups were compared.Logistic regression analysis was used to analyze the relationship between IL-6,25(OH)D,TNF-α and bronchial asthma complicated with MP infection.The receiver operating characteristic(ROC)curve was used to evaluate the predictive value of IL-6,25(OH)D and TNF-α for disease.Results:The levels of IL-6 and TNF-α in the infection group were higher than those in the non infection group and control group,and the infection rate in non infection group was significantly higher than that in the control group(all P<0.05).The level of 25(OH)D in the infection group was significantly lower than that in the non infection group and the control group,and that in the non infection group was significantly lower than that in the control group(all P <0.05).There was no significant difference in FVC,PEF and FEV1 among three groups(all P>0.05).Logistic regression analysis showed that IL-6,25(OH)D,TNF-α were independent risk factors for asthma complicated with MP infection(all P<0.05).The results of ROC curve analysis showed that ACU of IL-6,25(OH)D,TNF-α were above 0.7,which had a high predictive value for MP infection in bronchial asthma.TNF-α in infected group was positively correlated with IL-6(r=0.969,P<0.001),and TNF-α was positively correlated with 25(OH)D(r=0.984,P<0.001),and IL-6 was positively correlated with 25(OH)D(r=0.920,P<0.001).Conclusion:IL-6,25(OH)D,TNF-α are closely related to MP infection in asthma,which can be used as an important indexes to evaluate the degree of MP infection in asthma.
