摘要:
目的:探讨彩色多普勒超声联合血清学指标检测在先兆流产诊断及预后评估中的价值。方法:选取先兆流产患者110例为研究对象,将治疗后继续妊娠者纳入继续妊娠组(62例),将已确诊为先兆流产且后期发展为难免流产导致停止妊娠者纳入妊娠失败组(48例)。另选同期产检正常的健康孕妇50例为对照组。比较三组血清学指标[激活素A、抑制素A和糖类抗原125(CA125)]及彩色多普勒超声指标[黄体血流灌注指数(PI)和阻力指数(RI)]。利用受试者工作特征(ROC)曲线分析血清学及彩色多普勒超声指标单项和联合检测在先兆流产诊断及预后评估中的价值。结果:继续妊娠组和妊娠失败组孕妇血清激活素A、抑制素A水平低于对照组,CA125、黄体血流PI、黄体血流RI高于对照组(均P<0.05)。妊娠失败组孕妇激活素A、抑制素A低于继续妊娠组,CA125、黄体血流PI、黄体血流RI水平高于继续妊娠组(均P<0.05)。ROC曲线分析结果显示,激活素A、抑制素A、CA125、黄体血流PI、黄体血流RI单项诊断先兆流产及评估预后的AUC均高于0.7,且联合检测的曲线下面积(AUC)高于各单项(均P<0.05)。结论:血清激活素A、抑制素A、CA125、黄体血流PI、黄体血流RI对先兆流产的诊断及预后评估有一定的价值,且联合检测具有更高的效能。
Abstract:
Objective:To explore the value of color Doppler ultrasound combined with serological indexes detection in the diagnosis and prognosis evaluation of threatened abortion.Methods:110 patients with threatened abortion were selected as the research objects.The patients with continued pregnancy after treatment were included in the continued pregnancy group(62 cases),and those who had been diagnosed with threatened abortion and stopped pregnancy in the late stage due to inevitable abortion were included in the failed pregnancy group(48 cases).Another 50 healthy pregnant women with normal antenatal checkups during the same period were selected as the control group.The three groups of serum indexes(activin A,inhibin A and CA125)and color Doppler ultrasound indexes(PI and RI of corpus luteum)were compared.The ROC curve was used to analyze the value of single and combined detection of serological and color Doppler ultrasound indexes in the diagnosis and prognosis evaluation of threatened abortion.Results:The serum activin A and inhibin A levels of pregnant women in the continued pregnancy group and failed pregnancy group were lower than those in the control group,and CA125,PI and RI of corpus luteum were higher than those in the control group(all P<0.05).The levels of activin A and inhibin A in the failed failure group were lower than those in the continued pregnancy group,and the levels of CA125,PI and RI of corpus luteum were higher than those in the continued pregnancy group(all P<0.05).ROC curve analysis results showed that the AUC of single activin A,inhibin A,CA125,PI and RI of corpus luteum for diagnosis of threatened abortion and prognosis assessment were all higher than 0.7,and the AUC of the combined detection was higher than that of single detection(all P<0.05).Conclusion:Serum activin A,inhibin A,CA125,PI and RI of corpus luteum have certain value in the diagnosis and prognosis assessment of threatened abortion,and the combined detection has higher efficiency.
