摘要:
目的:探讨自体血液浓缩物用于侧壁开窗式上颌窦底提升术后患者的临床效果。方法:选择上颌后牙区种植修复的患者76例为研究对象,根据随机信封抽签原则把患者分为研究组与对照组各38例。对照组给予侧壁开窗式上颌窦底提升术治疗,研究组在对照组治疗的基础上给予自体血液浓缩物治疗,记录与随访两组的预后情况。结果:研究组术后1个月、3个月的自觉症状评分均低于对照组(均P<0.05); 两组术后3个月的骨高度与骨宽度均高于术前1 d(均P<0.05),研究组高于对照组(P<0.05); 研究组术后3个月的牙周愈合位点数高于对照组、替代性吸收位点位点数低于对照组(均P<0.05); 两组术后3个月的种植体都存活,研究组的黏膜穿孔、牙周感染、种植体松动等并发症发生率为2.6%,低于对照组的23.7%(P<0.05)。结论:自体血液浓缩物用于侧壁开窗式上颌窦底提升术后患者可促进牙周愈合,降低并发症的发生,也能改善患者的自觉症状,提升骨高度与骨宽度。
Abstract:
Objective:To investigate the clinical effect of autologous blood concentrate used for maxillary sinus floor elevation with sidewall fenestration.Methods:76 patients for implant restoration in the posterior maxillary area were selected as the research objects.All the cases were divided into the study group and control group according to the principle of random envelope drawing,with 38 cases in each groups.The control group was treated with side wall fenestration maxillary sinus floor elevation,and the study group was treated with autologous blood concentrate on the basis of the control group.The prognosis of the two groups were recorded and followed up.Results:The scores of self-conscious symptoms at 1 and 3 months after operation in the study group were lower than those in the control group(all P<0.05).The bone height and bone width of the two groups at 3 months after operation were higher than those of the preoperative 1 day,and the study group was higher than the control group(all P<0.05).The number of periodontal healing sites at 3 months after operation in the study group was higher than that in the control group,and the number of alternative absorption sites was lower than that in the control group(all P<0.05).The implants in both groups were survived at 3 months after surgery,the incidence of complications such as mucosal perforation,periodontal infection,and implant loosening in the study group was 2.6%,which was lower than 23.7% in the control group(P<0.05).Conclusion:The use of autologous blood concentrate for patients after the maxillary sinus floor elevation with sidewall fenestration can promote periodontal healing,reduce the occurrence of complications,improve patients' symptoms,and increase bone height and bone width.
