摘要:
目的:探讨心外膜脂肪组织厚度预测2型糖尿病(T2DM)合并冠心病(CHD)患者心脑血管事件发生的价值。方法:回顾性分析84例T2DM合并CHD患者的临床资料,其中随访结果出现脑血管不良事件患者(记为心脑血管组)33例,未出现心脑血管不良事件患者(记为非心脑血管组)51例。对比两组糖脂代谢指标、心外膜脂肪组织厚度及冠状动脉病变积分,分析T2DM合并CHD患者发生心脑血管事件的影响因素,分析心外膜脂肪组织厚度预测T2DM合并CHD患者心脑血管事件发生的价值。结果:心脑血管组空腹血糖(FPG)、餐后2 h血糖(2hPPG)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)、甘油三酯(TG)、总胆固醇(TC)水平均高于非心脑血管组(均P<0.05),心脑血管组空腹胰岛素(FINS)水平低于非心脑血管组(P<0.05)。心脑血管组心外膜脂肪组织厚度、冠状动脉病变积分均高于非心脑血管组(均P<0.05)。Pearson方法分析显示心外膜脂肪组织厚度与冠状动脉病变积分呈正相关(r=0.804,P<0.05)。多因素分析显示FPG、FINS、心外膜脂肪组织厚度、冠状动脉病变积分是T2DM合并CHD患者发生心脑血管事件的影响因素(均P<0.05)。ROC曲线分析心外膜脂肪组织厚度预测T2DM合并CHD患者心脑血管事件发生的曲线下面积为0.796(95%CI:0.600~0.823)。结论:T2DM合并CHD患者心外膜脂肪组织厚度与冠状动脉病变程度关系密切,且心外膜脂肪组织厚度对T2DM合并CHD患者心脑血管事件的发生具有较高的预测价值。
Abstract:
Objective:To explore the value of epicardial adipose tissue thickness in predicting the occurrence of cardiovascular and cerebrovascular events in patients with type 2 diabetes(T2DM)and coronary heart disease(CHD).Methods:The clinical data of 84 patients with T2DM and CHD were retrospectively analyzed,including 33 patients with cerebrovascular adverse events(recorded as the cardio-cerebrovascular group)and 51 patients without cardio-cerebrovascular adverse events(recorded as non-cardio-cerebrovascular group).The glucose and lipid metabolism indexes,epicardial fat tissue thickness and coronary artery lesion scores of the two groups were compared.The influencing factors of cardiovascular and cerebrovascular events in patients with T2DM and CHD were analyzed.The value of epicardial adipose tissue thickness in predicting the occurrence of cardiovascular and cerebrovascular events in T2DM patients with CHD was analyzed.Results:Fasting blood glucose(FPG),2-hour postprandial plasma glucose(2hPPG),glycosylated hemoglobin(HbA1c),insulin resistance index(HOMA-IR),triglycerides(TG),and total cholesterol(TC)levels in the cardio-cerebrovascular group were higher than those in non-cardio-cerebrovascular group,and the fasting insulin(FINS)level was lower than that in non-cardio-cerebrovascular group(all P<0.05).The thickness of epicardial adipose tissue and the score of coronary artery disease in the cardio-cerebrovascular group were higher than those in non-cardio-cerebrovascular group(all P<0.05).Pearson analysis showed that the thickness of epicardial adipose tissue was positively correlated with coronary artery lesion score(r=0.804,P<0.05).Multivariate analysis showed that FPG,FINS,epicardial adipose tissue thickness,and coronary artery lesion score were the influencing factors of cardiovascular and cerebrovascular events in patients with T2DM and CHD(all P<0.05).ROC curve analysis showed that the area under the curve for the thickness of epicardial adipose tissue to predict the occurrence of cardiovascular and cerebrovascular events in patients with T2DM and CHD was 0.796(95% CI:0.600-0.823).Conclusion:Epicardial adipose tissue thickness is closely related to the severity of coronary artery disease in patients with T2DM and CHD.The epicardial adipose tissue thickness has high predictive value for the occurrence of cardiovascular and cerebrovascular events in patients with T2DM and CHD.
