摘要:
目的:探究miRNA-126和miRNA-92a在冠心病(CHD)患者血清中表达水平及临床价值。方法:选择56例CHD患者和30例健康体检者为对象。测定两组血清脂代谢指标、细胞因子、miRNA-126和miRNA-92a含量,Gensini评分法评估两组冠脉病变程度。ROC分析血清miRNA-126和miRNA-92a在CHD诊断和冠脉中重度病变预测中的价值。结果:与对照组比,观察组血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、血管细胞黏附分子-1(VCAM-1)和miRNA-92a含量明显增加(均P<0.01),HDL-C、Krüppel 样转录因子2(KLF2)、miRNA-126含量明显减少(均P<0.01)。血清miRNA-126含量与KLF2呈显著正相关,与VCAM-1、CXCL-12含量和Gensini评分呈显著负相关(均P<0.01); miRNA-92a含量与KLF2呈显著负相关(P<0.01),与VCAM-1含量和Gensini评分呈显著正相关(P<0.01)。ROC曲线分析显示,CHD诊断中血清miRNA-126和miRNA-92a的AUC为0.962和0.923,敏感度90.0%和85.7%,特异度82.1%和93.3%; 冠脉中重度病变预测中血清miRNA-126和miRNA-92a的AUC分别为0.897和0.866,敏感度93.5%和80.0%,特异度84.0%和90.3%。结论:CHD患者血清中miRNA-126表达水平降低,miRNA-92a表达水平升高,且其表达水平变化与冠脉病变程度显著相关,可作为CHD诊断及病情进展状况预测的可靠指标。
Abstract:
Objective:To explore the expression levels of miRNA-126 and miRNA-92a in the serum of patients with coronary heart disease(CHD)and their clinical value.Methods:56 CHD patients and 30 healthy subjects were selected as subjects.The levels of serum lipid metabolism indexes,cytokines,miRNA-126 and miRNA-92a were measured in the two groups,and the degree of coronary artery disease in the two groups was assessed by Gensini scoring method.ROC curve was used to analyze the value of serum miRNA-126 and miRNA-92a in the diagnosis of CHD and the prediction of moderate and severe coronary lesions.Results:Compared with the control group,the serum TC,TG,LDL-C,VCAM-1 and miRNA-92a levels in the observation group were significantly increased(all P<0.01),and HDL-C,KLF2,and miRNA-126 levels were significantly reduced(all P<0.01).Serum miRNA-126 content was significantly positively correlated with KLF2,and significantly negatively correlated with VCAM-1,CXCL-12 content and Gensini score(all P<0.01).Serum miRNA-92a content was significantly negatively correlated with KLF2(P<0.01),and significant positively correlated with VCAM-1,CXCL-12 content and Gensini score(all P<0.01).ROC curve analysis showed that the AUC of serum miRNA-126 and miRNA-92a in CHD diagnosis were 0.962 and 0.923,the sensitivity was 90.0% and 85.7%,the specificity was 82.1% and 93.3%; the AUC of serum miRNA-126 and miRNA-92a in the prediction of moderate to severe coronary lesions were 0.897 and 0.866,the sensitivity was 93.5% and 80.0%,the specificity was 84.0% and 90.3%.Conclusion:The expression level of miRNA-126 in the serum of CHD patients is decreased,and the expression level of miRNA-92a is increased,and their expression levels are significantly correlated with the degree of coronary artery disease,which can be used as a reliable indicator for CHD diagnosis and disease progression prediction.
