摘要:
目的:探讨Osterix蛋白在关节软骨中的表达强度对膝关节骨性关节炎形态学变化的影响。方法:选取44例因膝关节骨性关节炎行人工全膝关节置换术的患者,取术中去除的股骨髁软骨组织,磨损较重的一侧为负重区(实验组),磨损较轻的一侧为非负重区(对照组)。通过HE染色评估软骨组织形态学变化,再将负重区软骨分为中度磨损组和重度磨损组,通过免疫组织化学检测并比较Osterix蛋白在三组中的表达强度。结果:大体观察下,负重区标本表面粗糙,大量软骨丢失,软骨下骨板暴露,色泽灰暗,甚至可见溃疡灶形成; 非负重区标本表面光滑平整,富有光泽,软骨完整且呈乳白色,未见溃疡灶。通过HE染色后可见负重区软骨面残缺不齐,过渡层出现裂隙,软骨细胞分布紊乱,潮线不完整,软骨下骨排列不均; 非负重区组软骨表面较为平整,软骨细胞数量多,胞质完整均匀。免疫组化染色可见Osterix蛋白在非负重区软骨中显著表达,在负重区软骨表达较少。统计学分析显示,重度磨损组Osterix蛋白阳性表达率小于中度磨损组,且两者均小于非负重区组(均P<0.05)。结论:Osterix蛋白参与了关节软骨退行性变过程,其在软骨细胞中的表达强度会随着软骨磨损程度的增加而减弱。
Abstract:
Objective:To investigate the effect of the expression intensity of Osterix protein in articular cartilage on the morphological changes of knee osteoarthritis.Methods:44 patients who underwent total knee arthroplasty for osteoarthritis of the knee were selected as research objects.The femoral condylar cartilage tissue removed during the operation was taken.The side with heavy wear was the weight-bearing area(experimental group),and the side with light wear was the non-weight-bearing area(control group).Cartilage histomorphology was evaluated by HE staining,and the load-bearing area cartilage was then divided into the moderate wear group and the severe wear group.Immunohistochemistry was used to detect and compare the expression intensity of Osterix protein in cartilage tissues with different degrees of wear in the three groups.Results:The surface of the specimens in the weight-bearing area was rough,a large number of cartilage was lost,the subchondral bone plate was exposed,the color was gray,and even ulcers were visible; the surface of the specimens in the non weight-bearing area was smooth,glossy,the cartilage was complete and milky white,and no ulcers were found.HE staining showed that the weight-bearing area had incomplete cartilage surface,cracks in the transition layer,disordered distribution of chondrocytes,incomplete tidal line,and uneven arrangement of subchondral bone.The cartilage surface of the non-weight-bearing area was relatively flat,the number of chondrocytes was large,and the cytoplasm was intact and uniform.Immunohistochemistry staining showed that Osterix protein was significantly expressed in the cartilage of the non-weight-bearing area,but the expression was less in the cartilage of the weight-bearing area.Statistical analysis showed that the positive expression rate of Osterix protein in the severe wear group was lower than that in the moderate wear group,and both were lower than that of the cartilage of the control group(all P<0.05).Conclusion:Osterix protein is involved in the process of articular cartilage degeneration,and its expression intensity in chondrocytes will decrease as the degree of cartilage wear increases.
