摘要:
目的:比较T1-mapping、T2-mapping、T2*-mapping三种MRI生理学定量技术在早期膝关节软骨损伤中的诊断价值。方法:纳入52例行关节镜手术的患者,将国际软骨修复协会软骨损伤分级系统(ICRS)分级为0级损伤者作为对照组,将1、2级损伤者作为早期损伤组。所有患者于术前行患侧膝关节T1-mapping、T2-mapping及T2*-mapping序列及常规序列MRI扫描。采用受试者工作特征(ROC)曲线对三者的诊断效能进行评价。结果:早期损伤组T1、T2、T2*值均高于对照组(均P<0.05)。ROC曲线分析结果显示,三种技术对早期膝关节软骨损伤均有较高的诊断效能(均P<0.05),曲线下面积(AUC)由高至低依次为T2*-mapping、T2-mapping、T1-mapping,且T2*-mapping序列的敏感度优于另外两个序列。结论:磁共振T1-mapping、T2-mapping及T2*-mapping序列均可用于早期膝关节软骨损伤的定量诊断,其中T2*-mapping的诊断价值更高。
Abstract:
Objective:To campare the diagnostic value of T1-mapping,T2-mapping and T2*-mapping for early cartilage injury of knee joint.Methods:52 patients underwent arthroscopic surgery were included,and those with grade 0 injury in ICRS were classified as the control group,and those with grade 1 and 2 injuries were regarded as the early injury group.All patients underwent T1-mapping,T2-mapping,T2*-mapping and routine MRI of knee joint before surgery.The diagnostic efficacy of the three quantitative techniques was evaluated by ROC curve.Results:The T1,T2 and T2* values of the early injury group were higher than those of the control group(all P<0.05).ROC curve analysis results showed that the three techniques had high diagnostic efficiency for early cartilage injury of knee joint(all P<0.05),and the AUC from high to low was T2*-mapping,T2-mapping,T1 -mapping,and the sensitivity of the T2*-mapping was better than the other two sequences.Conclusion:T1-mapping,T2-mapping and T2*-mapping sequences can be used for the quantitative diagnosis of early cartilage injury of knee joint,and T2*-mapping has a higher diagnostic value.
